Özet
Amaç: Arka kapsüloreksis (AK) uygulanan hastalarımızda ameliyat sırasında ve sonrasında meydana gelen komplikasyonları ve cerrahi sonuçlan retrospektif olarak analiz etmek. Gereç ve Yöntem: Çalışmada fakoemülsifikasyon veya lens aspirasyonu yapılan 67 olgunun 82 gözüne AK uygulandı. Olgular en düşük 6 aylık e$ yüksek 75 yaşında olup ortalama yaş 36 idi. 17 olgunun 22 gözü konjenital katarakt, 8 olgunun 16 gözü yüksek miyoplu katarakt, 4 olgunun 4 gözü üveitik katarakt, 38 olgunun 40 gözü senil katarakt sebebiyle ameliyat edildi. Ameliyat öncesi ve sonrası muayenelerde; biometri, görme keskinliği, biomikroskopi, applanas-yon ile göz içi basıncı ölçümü (GİB), arka kapsül kesifliği (AKK), ön kamarada vitre, fibrin mevcudiyeti, YAG lazer, retina dekolmanı ve klinik makuler ödem varlığı kontrol edildi. Bulgular: Olgularımızın 23'ünde genel anestezi, 39'unda subtenon, 5'inde topikal anestezi uygulandı. 25 gözde lens aspirasyonu ile AK, 1 gözde lens aspirasyonu + AK + ön vitrektomi, 44 gözde fakoemülsifikasyon + AK, 12 gözde fakoemülsifikasyon + AK + ön vitrektomi uygu¬landı (Tablo 1). Ortalama takip süremiz 13 ay olup en az 6 ay en fazla 2.5 yıl idi. Üç gözde dü¬zensiz AK oluştu. Bir gözde korneal keşi yerine uzanan vitre bandı mevcuttu. Beş yaş altı pedi-atrik grupta vitrektomi uygulanmamış iki gözde lens arkasında kapsül kesifliği gelişti (AKK). Bir göze YAG lazer uygulandı, 9 gözde postoperatif ilk bir ayda fibrin mevcuttu. Hiçbir olguda retina dekolmanı ve klinik makuler ödem oluşmadı. Vizüel prognoz açısından 75 gözde görme keskinliğinde artış olurken beş gözde görme değişmedi. İki üveitik gözde bir yıl içinde optik at-rofi sebebiyle görme keskinliğinde düşüş gözlendi. İki olguda geçici postoperatif GIB yüksekliği oldu. Yorum: AK uygulaması biraz tecrübe gerektirmekle beraber, AKK açısından oldukça iyi sonuçlar vermektedir:1 Pediatrik yaş grubunda katarakt cerrahisinin rutinleri arasına girmektedir. Takip süremiz kısa olmakla beraber komplikasyonların azlığı dikkat çekmektedir.
Abstract
Purpose: Retrospective evaluations of per-op complications and surgical results of patient in whom posterior capsulorhexis (PCCC) procedure employed. Material and Method: In our study PCCC procedure was employed in 82 eyes of 67 pati¬ents, who have undergone phacoemulcification and lens aspirating operation. The youngest pati¬ent was 6 months and the oldest one 75. The mean age was 36.22 eyes of 17 patients were diag¬nosed as congenital cataract, 16ieyes of 8 patients high myopia, 4 eyes of 4 patients cataract due to uveitis. 40 eyes of 38 patients were senile cataract. Pre-op and post-op investigations were bio¬metry, visual acuty, biomicroscopy, IOP by applanation, posterior capsuler opacification, vitreus presence in anterior chamber, fibrine presence, YAG laser, retinal detachment and presence of clinical maculer edema. Results: General anesthesia employed to 23 patients subtenon anesthesia to 39 and topical anesthesia to 5. The details of operations were as follows; Lens aspiration and PCCC 25 eyes, lens aspiration+PCCC+ anterior vitrectomy in 1 eye, Phacoemulcification+PCCC in 44 eyes, phacoemulcification+PCCC+ anterior vitrectomy in 12 eyes. Mean follow up time 13 months, the shortest 6 months and the longest 2.5 years. Irreguler PCCC has occured in 3 eyes. In one eye there was a vitreus band extending corneal incision side. In 2 eyes without vitrectomy in under 5 years old pediatric group has developed posterior capsule opacification. YAG laser has done to 1 eye. Fibrine has occur in 9 eyes in post-op 1st month. Retinal detechment and clinical macular edema has not been observed in any patient. Visual acuty increased in 75 patients but didn't dif¬fer 5 patients. Two eyes with uveitis visual acuty has decreased in post-op 1 year due to optic nerve atrophy. In 2 patients there was transient IOP increase. Conclusion: Although it requires experience of surgeon, the PCCC procedure gives superior results. It becomes currently a routine practice in cataract surgery in pediatric populations. Even though our follow up was short. We still could emphasize rarity of complications in this study.
