Fakoemulsifikasyonda kullanılan farklı konsantrasyonlardaki Na-hyaluronatın postoperatif göz içi basıncına etkileri

Özet
Amaç: Fakoemulsifikasyonda kullanılan farklı iki konsantrasyondaki Na-hyaluronat' in postoperatif erken dönemde göz içi basıncına etkilerinin değerlendirilmesi. Materyal-Metod: Lokal anestezi altında fakoemülsifikasyon uygulanan 169 olgu çalışma kapsamına alındı. Olguların 91'inde (%54), %1.4'lük Na-hyaluronat (Grup I); 78'inde (%46) %1'lik Na- hyaluronat (Grup II) içeriğindeki viskoelastik madde kullanıldı. Tüm olgular aynı cerrahi teknikle ameliyat edildi ve postoperatif ilk 24 saat içinde, 1. gün, 1. hafta, 1. ay ve 3. ayda rutin olarak muayene edildi. Postoperatif dönemde GİB değişiklikleri ve buna etki eden fak¬törler incelendi. Bulgular: Grup I'de ortalama preoperatif GİB 14.06±3.8 mmHg (min 8 mmHg, mak 24 mmHg), Grup II1 de ortalama preoperatif GİB 14.77±4.61 mmHg (min 8 mmHg, mak 36 mmHg) olup her iki grubun preoperatif GİB' leri arasında fark yoktu (p>0.05). Olguların postoperatif ilk 24 saat içindeki GİB' lan, Grup I'de 16.01 ± 8.78mm Hg (min 5 mmHg, mak 44 mmHg) olup, Grup II' de ortalama 15.40±8.87 mmHg (min 2 mmHg, mak 48 mmHg) olarak saptanmış olup fark istatistiksel olarak anlamsızdı (p>0.05). Grup I'de 35 olguda (%38.46) pos¬toperatif GİB düşme gösterirken; Grup Il'de 43 olguda (%55.12) GIB'ında düşme saptandı, fark istatistiksel olarak anlamsızdı (p>0.05). Grup I'de 44 olguda (%48.3) ameliyat sonrası ilk 24 sa¬at içinde GİB'de yükselme olurken, Grup Il'de 32 olguda (%41.02) GİB yüksek olarak ölçüldü. Olguların 24'ünde (% 12.24), postoperatif 22 mmHg üstünde GİB saptandı. Grup I'de GİB 22 mmHg ve daha fazla olan 18 olgu (% 19.78) varken Grup Il'de 6 olguda (%7.69) bu düzeyde GİB yükselmesi saptandı. Fark istatistiksel olarak anlamsızdı (p>0.05). Sonuç: Katarakt cerrahisi sonrası, GİB yükselmesi ameliyatı takip eden erken dönemde görülebilmekte, farklı yapı ve konsantrasyondaki viskoelastik maddelerle GÎB yükselme düzeyi farklı olabilmektedir.
Abstract
Purpose: To evaluate the effect of Na-hyaluronate with two different concentration on postoperative intraocular pressure. Material and method: In this study, 169 eyes underwent phacoemulsification surgery un¬der local anesthesia are evaluated. On 91 ofeyes (54%), 1.4% Na-hyaluronate (Group I) and on the remainder 78 eyes (46%), 1% Na-hyaluronate (Group II) were used. All the subjects were operated with the same surgical procedure and examined on the 1st postoperative day, 1st week, 1st month and 3rd month. Postoperative intraocular pressure (IOP) changes and the effecting factors were investigated. Results: The mean preoperative IOPs in Group I and Group II were 14.06±3.8 mmHg (min 8 mmHg, max 24 mmHg) and 14.77±4.61 mmHg (min 8, max 36 mmHg) respectively and the¬re was no statistically significant difference (p> 0.05). The IOP's of the subjects in first 24 pos¬toperative hours were 16.01±8.78 mmHg (min 5 mmHg, max 44 mmHg) in Group I and 15.40±8.87 mmHg (min 2mm Hg, max 48 mmHg) in Group II and the difference was not statis¬tically significant. Postoperative IOP decreased in 35patients (38.46%) in Group I, and in 43 pa¬tients (55.12%) in Group II (p>0.05). Raised IOPs were found in 44 eyes (48.3%) in postopera¬tive 24 hours in Group I and in 32 eyes (41.02%)in Group II (p>0.05). In 24 of the patients (% 12.24), IOP was measured over 22 mmHg. Number of patients with 22 mmHg or more IOP levels were 18 (19.78%) in Group I, and 6 (7.69%) in Group II and the difference was statisti¬cally insignificant (p>0.05). Conclusion: After cataract surgery, IOP rise can be seen in early postoperative period and IOP rise levels may be different with viscoelastics with different concentrations.
