Özet

Amaç: Komplet tip Vogt-Koyanagi-Harada Hastalığı tanısı almış bir olgunun klinik özelliklerini ve tedavi yaklaşımlarını gözden geçirmektir. Yöntem: GATA Haydarpaşa Eğitim Hastanesi GQZ Kliniğinde, her iki gözde görme azlığı nedeniyle Enfeksiyon Hastalıkları Kliniği tarafından konsültasyonu istenen 21 yaşındaki erkek hasta Vogt-Koyanagi-Harada hastalığı açısından değerlendirildi. Bulgular: Enfeksiyon Hastalıkları Kliniğinde baş ağrısı, subfebril ateş ve uykuya meyil şikayetleri ile menenjit ön tanısı konularak takip edilen hastada, her iki gözde görme azlığı gelişmesi üzerine yapılan oftalmolojik muayenede; sağ gözde ön üveit ve vitritis, sol gözde iridosiklitis saptandı. Görme keskinliği Snellen eşeline göre sağ gözde 0.3, sol gözde 0.7 seviyesindeydi. Üveitin başlangıcından önce penetran göz travması veya cerrahi hikayesi olmaması, diğer oküler hastalık antitelerinin varlığını telkin edecek klinik ve labaratuvar bulgularının olmaması, çift taraflı göz tutulumu, menenjismus ve beyin omurilik sıvısında lenfosit ağırlıklı pleositozis, geç dönemde gelişen alopesi ve poliozis varlığı ile komplet tip Vogt-Koyanagi-Harada hastalığı tanısı konuldu. Tedaviye intravenöz olarak gün aşırı 1000 mg metilprednizolon ile başlanılıp, üç uygulamayı takiben 100 mg/gün oral prednizolon idamesine geçildi ve bu tedaviye doz tedrici olarak azaltılarak 10 ay devam edildi. En son kontrol muayenesinde (10 ay sonra) görme keskinliği sağ gözde 0.8, sol gözde tam idi. Sonuç: Koroid ve diğer melanositlere karşı oluşan aşırı duyarlılık veya otoimmun bir enflamasyon olduğu kabul edilen bu hastalığın, Vogt-Koyanagi-Harada tanı kriterlerine göre komplet, inkomplet ve olası tip olmak üzere 3 tipi mevcuttur ve günümüzde enflamasyon şiddetinin ve klinik belirtilerinin farklılığına bağlı olarak ortaya çıkan tek bir hastalık olduğu kabul edilmektedir.

Abstract

Purpose: To report a case diagnosed as complete type Vogt-Kayanagi-Harada Disease and to evaluate its treatment approaches. Methods: A 21 years-old man with bilateral decrease of visual acuity referred by the Department of Infectious Diseases was evaluated and diagnosed as Vogt-Kayanagi-Harada Disease in Ophthalmology Department of GATA Haydarpaşa Trainnig Hospital. Results: A patient complaining of headache, somnolence and sub-febrile fever who was followed in the department of infectious diseases with the diagnosis of meningitis was referred to our clinic due to sudden decrease of visual acuity. The ocular examination showed that the patient had anterior uveitis and vitritis in the right eye and iridocyclitis in the left eye. Visual acuities were 0.3 in the right and 0.7 in the left eye. We diagnosed the case as complete type of Vogt-Kayanagi-Harada disease because of bilateralism, meningismus, alopecia, poliozis, and the absence of penetrating trauma or surgery history, clinical and laboratory findings of other ocular diseases. The therapy was started with intravenous 1000 mg methylprednisolone every other day. After three intravenous methylprednisolone injections, we continued with 100 mg/day oral prednisolone, and tapered the dose during 10 months. We measured visual acuities as 0.8 in the right and 1.0 in the left eye at the follow-up visit of 10 months. Conclusion: Vogt-Kayanagi-Harada Disease, which has three clinical types (complete, incomplete and possible type), has been described as an autoimmune systemic disease or hyper-sensitivity against melanocytes. We reported a case of incomplete type disease which is diagnosed rarely.
