Özet
Amaç: Oküler toksokariazis tanısı alan bir olgumuzun klinik özelliklerini ve laboratuvar bulgularını değerlendirmek. Yöntem: GATA Haydarpaşa Eğitim Hastanesi Göz Kliniğine tek taraflı görme azlığı ve ekzotropya ile başvuran 20 yaşındaki erkek hastada oküler toksokariazis araştırıldı. Bulgular: Sol gözde görme keskinliği 1 metreden parmak sayar (mps) seviyede olup, bu gözde primer bakış pozisyonunda Hirscberg testi ile yaklaşık 30 derece ekzotropya mevcut idi. Biyomikroskopik muayenede sol gözde vitreusta pigmente inaktif hücreler, oftalmoskopik muayenede sol gözde optik diskin üst orta periferinde yaklaşık 3 disk çapında, aktif olmayan, korioretinada beyaz renkli granülomatöz kitle ve buradan çevresindeki retinaya ve optik diske doğru uzanım gösteren traksiyonel vitreus bantları ile yer yer vitreoretinal membranlar ve makula deliği mevcut idi. Sağ göz görmesi tam olup biyomikroskopik ve oftalmaskopik muayeneleri doğal idi. Flörosein anjiografide (FA) granülomda erken dönemde başlayıp geç dönemlerde giderek artan hiperflöresans izlendi. Serolojik incelemede ELISA ve Western Blotting testleri yapıldı, her iki testte oküler toksokariazis için pozitif idi. Sonuç: Köpek ve kedilerin bağırsaklarında sık olarak bulunan Toksocara canis ve Tokso-cara catis larvalarının ağız yoluyla alınmalarını takiben, kısa silier arterler veya santral retina arteri yoluyla göze girmeleri ile gelişen oküler toksokariazisin sıklığı bilinmemekle birlikte, çocuk ve genç yaştaki insanlarda önemli bir görme azlığı nedenidir. Kedi-köpek topluluklarının fazla ve hijyenik şartların yetersiz olduğu ülkemizde beklenilenin aksine bildirilmiş çok az olgunun olması nedeniyle bu hastalığın üzerinde durulması gerekliliğini düşünmekteyiz.
Abstract
Purpose: To describe clinical characteristics and laboratory findings of a case with ocular toxocariasis. Methods: A man of 20 years old presented with unilateral low vision and exotropia was evaluated for ocular Toxocara infection in the Department of Ophthalmology, Haydarpaşa Training Hospital, Gulhane Military Medical Academy. Results: The visual acuity of patient's left eye was measured as counting finger at 1 meter. An exotropia of 30° was detected using Hirschberg 's light reflex test in primary gaze. Pigmented inactive cells were present in the left eye's vitreous in biomicroscopic examination. In fundus examination, an inactive whitish chorioretinal granuloma with a width of 3 disc diameters was found in the superior midperiphery of the left eye. A macular hole was seen. There were vitreous traction bands extending from the granuloma to the optic disc, and the retina neighboring the lesion associated with vitreoretinal membranes. A hyperfluorescence beginning at the early phase and increasing at-the late phase was showed in fluorescein angiography. Results of the enzymelinked immunoabsorbent assay (ELISA) and the Western Blotting analysis were positive for toxocariasis. Conclusion: Toxocara canis and catis, nematods living in the dog's and cat's intestines, larvas infuse the eye via short ciliary arteries and central retinal artery after oral ingestion, can cause severe chorioretinitis, resulting with vision decrease in children and young people. Despite the insufficient hygienic conditions and high population of unappropriated pets in our country, reported toxocariasis cases are few. Reporting this case, we aimed to attract attention of our colleagues to toxocariasis.
