Primer pterjium cerrahi tedavisinde konjunktival Z- plasti yönteminin etkinliğini araştırmak

Özet
Amaç: Bizim çalışmamızın amacı primer pterjium cerrahi tedavisinde rekürrensi azaltma yönünden, konjunktival Z-plasti yönteminin çıplak sklera yöntemiyle karşılaştırılması idi. Yöntem: Kasım 2002- Kasım 2003 tarihleri arasında Ankara Eğitim Araştırma Hastanesi Göz polikliniğimize başvuran ve primer pterjium tanısı alan 45 hastanın 50 gözü çalışmaya alındı. Hastaların 25'ine (20 kadın, 5 erkek) çıplak sklera yöntemi (Grup I), 25'ine ise (18 kadın, 7 erkek) konjunktival Z-plasti yöntemi (Grup II) uygulandı. Vakaların tümü postoperatif 1.7. 15. gün ve 1,2,3,6 ay sonunda kontrol edildi. Limbusu lmm geçen korneal ilerlemeler nüks olarak kabul edildi. Bulgular: Grup I'de vakalar ortalama 10.5±2.3 ay (8-14 ay), Grup H'de ise ortalama 10.5±2.4 ay (8-15 ay) takip edildi. Grup I'de vakaların 14'ünde (%56) nüks izlenirken, Grup H'de vakaların 4'ünde (%12) nüks izlendi. Nüksler Grup I'de ortalama 2±0.7 ay, Grup H'de ortalama 2.25±0.3 ayda izlendi. Peroperatuar komplikasyon gelişmezken, postoperatif her iki grupta da % 12 oranında komplikasyon izlendi. Tartışma: Konjunktival Z-plasti tekniğinin primer pterjium cerrahisi için basit, kolay uygulanabilir, komplikasyonu az, nüksü önleme açısından efektif bir yöntem olacağı sonucuna vardık.
Abstract
Purpose: The aim of our study is to compare the recurrence rate of conjunctival Z-plasty and the bare sclera method in primary pterygium surgery. Method: Fifty eyes of forty five patients who were seen at the ophalmology department of the Ankara hospital were included in the study. The bare sclera method was done in the 25 of the patients (20 women, 5 men; Group 1) and conjunctival Z-plasty was done in the 25 patients (18 women, 7 men; Group 2). The patients were evaluated postorjeratively at 1st, 7th, 15th day and first, 2, 3 and 6 month. Corneal overgrowth more than lmm was accepted as recrrence. Results: The follow-up period was 10.5+2.3 (8-14) months in Group 1 and 10.5±2.4 (8-15) months in Group 2. The recurrence was seen in 14 (%56) patients in Group 1 and 4 (%12) patients in Group 2. The recurrence occured at the postoperative 2±0.7 months in Group 1 and 2.25±0.3 months in Group 2. No peroperative complication was seen in both groups and postoperative complications were seen in % 12 of the patients. Discussion: We purpose that conjunctival Z-plasty is a simple, effective and easily appli-caple method with fewer complications and lower recurrence rates in primary pterygium surgery.
