ETROP Kriterlerinin Dışında Yer Alan Bir Bebeğin Tedavisinde Binoküler İndirekt Oftalmoskop ve Nonkontakt Geniş Açılı Görüntüleme Sistemi Optos’un Kullanılması

Binocular Indirect Ophthalmoscopy Complements Non-contact Widefield Imaging with Optos to Treat a Baby Outside ETROP Guidelines

Özet

Bu olgu sunumunda 24 hafta ve 600 g ağırlığında doğan, erkek, prematüre bir bebek sunulmaktadır. Hastanın postmenstrüel 38.haftadaki muayenesinde, zon 2’de, evre 2 prematüre retinopatisi (PR) vardı ve artı hastalığı yoktu. Bir hafta sonraki muayenede çekilengeniş açılı Optomap görüntülerinde, hastalığın evresinde bir değişiklik olmadığı görüldü. Postmenstrüel 40. haftadaki muayenede zon2’de, evre 3 PR saptandı. Binoküler indirekt oftalmoskopi ve Optos görüntüleme cihazı, damarlarda çok az kıvrımlanma ve genişlemeile birlikte artı öncesi hastalığı belirledi. Bir hafta sonra, 41. postmenstrüel haftada Optomap görüntüleri nazal kadrandaki kötüleşenekstraretinal fibroproliferasyonları gösterdi. Sonuç olarak, bu bebek fundus lazer fotokoagülasyon ile tedavi edildi.

Abstract

We report a male premature baby who was born at 24 weeks gestation weighing 600 grams. There was bilateral stage 2, zone 2 retinopathy of prematurity (ROP) without plus disease at 38 weeks postmenstrual age. Ultra-wide-field Optomap images obtained 1 week later showed no change in ROP stage. At 40 weeks postmenstrual age, stage 3, zone 2 ROP was detected using binocular indirect ophthalmoscopy and documented using Optos. Minor tortuosity and dilation of vessels was interpreted as pre-plus disease. One week later, at 41 weeks postmenstrual age, Optomap images identified progressive extraretinal fibroproliferation in the nasal quadrant. As a result, the baby was treated with fundus laser photocoagulation.
