Orbita Kistine Bağlı Ensefalokraniyokutanöz Lipomatozis: Varyant mı Yoksa Yeni Bir Hastalık mı?

Özet
Ensefalokraniyokutanöz lipomatozis (EKKL), aynı zamanda Haberland veya Fishman sendromları olarak da bilinen, karakteristik olarak santral sinir sistemi, gözler ve deri gibi ekto mezo dermal dokuları tutan son derece nadir görülen bir konjenital nörokutanöz hastalıktır. Hastalığın etiyolojisi bilinmemektedir. Bu olgu sunumunda Sahra Altı Afrika’da bilateral göz tutulumu ve orbital kisti izlenen nadir bir EKKL olgusu sunulmuştur. Üç yaşında erkek hasta doğumdan beri mevcut olan sağ gözde kistik şişlik şikayeti ile başvurdu. Fizik muayenede kafa derisinin sağ tarafında alopesi, ipsilateral oküler kist ve kontralateral limbal dermoidin eşlik ettiği mikroftalmi saptandı. Beyin tomografisinde sağ serebral hemisferde ileri derecede atrofi, serebrospinal sıvı aralığında genişleme ve lateral ventrikülde hidrosefali ex-vacuo düşündüren dilatasyon saptandı. Bulbus ile devamlılık gösteren sağ orbita kisti ve her iki bulbusun arka kesiminde kalsifikasyon izlendi. Eksize edilen orbita kistinin histopatolojik incelemesinde yağ dokusu ile kaplı göz küresi, kist duvarında kalsifikasyon ve kornea opasitesi mevcuttu. Mikroskopik değerlendirme koristom ile uyumlu şekilde kornea sklera duvarının normal kıkırdak dokudan oluştuğunu, trabeküler kemik yapılar içerdiğini ve kemik iliği hücrelerinden oluşan bir odak bulunduğunu gösterdi. Bu bulgular, EKKL tanısı için 2009’da önerilen Moog tanı kriterleri ile uyumludur. Hastalık doğumda kolayca tanınabilse de, nörogörüntüleme; uygun tanı ve tedavinin yanı sıra diğer nadir görülen ilişkili anomalileri dışlamak veya doğrulamak için gereklidir.
Abstract
Encephalocraniocutaneous lipomatosis (ECCL), also known as Haberland or Fishman syndrome, is an extremely rare congenital neurocutaneous disorder that characteristically involves ectomesodermal tissues such as the central nervous system, eyes, and skin. The etiology of the disease remains unknown. Here we present a rare case of ECCL associated with bilateral eye involvement and orbital cyst from Sub-Saharan Africa. A 3-year-old boy presented with cystic right eye swelling since birth. Physical examination showed alopecia on right side of the scalp, ipsilateral ocular cyst, and microphthalmia with a contralateral limbal dermoid. Computed tomography of the brain revealed severe atrophy of the right cerebral hemisphere with an expansion of the cerebrospinal fluid space and dilatation of the lateral ventricle suggesting ex-vacuo hydrocephalus. Right orbital cyst continuous with the globe and calcification of the posterior aspect of both globes were also present. Histopathologic findings of the excised orbital cyst revealed an eyeball covered by fatty tissue, calcification of the cyst wall, and corneal opacity. Microscopy showed cornea-sclera wall composed of normal cartilage communicating with sandwich bony trabeculae with a focus of marrow cells, consistent with choristoma. The constellation of these findings conforms to Moog’s revised diagnostic criteria for ECCL proposed in 2009. Although the disorder is easily recognizable at birth, neuroimaging is essential for appropriate diagnosis and management and to exclude or confirm other unusual associated abnormalities.
