İnternal akustik kanal'ın nöroradyolojik ve suboksipital retrosigmoid girişim ile morfometrik analizinin karşılaştırılması: Anatomik çalışma

Özet
Giriş: Temporal kemik morfolojisindeki küçük varyasyonlar serebellopontin köşeye yapılan girişimler sırasında morbidite ve mortaliteyi etkileyen en önemli faktörlerdir. Yöntem: Çalışmamızda 20 kadaverik temporal kemiğin kemik penceresinde 1 mm lik bilgisayarlı tomografi (BT) kesitleri alındı. Birinci aşamada bu kesitlerde sigmoid sinüsün medialinden fundus'a çekilen hatta göre (S-F hattı); labirent lokalizasyonu ve girişim açıları, inferolateral internal akustik kanal'dan (İLİAK) fundus, kommun krus ve vestibül arası uzaklıkları mm olarak ölçüldü ve juguler bulb'ın yeri kayıt edildi. İkinci aşamada aynı kadavralara mikroşirurjikal teknikle suboksipital retrosigmoid girişim yapılarak KT'de saptanan ve mikroşirurjikal olarak tespit edilen değerlerle karşılaştırılmak üzere aynı ölçümler yapıldı. Bulgular: KT'de labirent lokalizasyonu; S-F hattının lateralinde %40 (n:8), üzerinde %35 (n:7) ve medialinde %25 (n:5) olarak saptandı. Örneklerin 2'sinde (%10) yüksek yerleşimli juguler bulb mevcuttu. BT'den ve mikroşirirjikal teknikle elde edilen İLİAK- fundus, İLİAK- kommon krus ve İLİAK- vestibül arası uzaklıklar karşılaştırıldığında istatistiksel fark saptanmadı (p>0,05). Labirenti S-F hattının medialinde olan 4 (%20) örnekte fundusun görülmesi oldukça geniş bir açı gerektirdi. Sonuç: internal akustik kanalı (İAK) tutan lezyonlara yönelik cerrahi girişimlerin planlanmasında, preoperatif kemik penceresinde çekilen temporal kemik KT sinden elde edilecek bilgiler morbiditeyi en aza indirmeye yardımcı olacaktır;
Abstract
Introduction: A little variations of the morphology of the temporal bone are the most important factors that have effects on morbidity and mortality during surgical procedures at the cerebellopontine angle. Methods: In the present study, axial bone window computerized tomography (CT) slices of 20 adult cadaveric temporal bones were obtained. As an initial step according to a line that is drawn from the medial of the sigmoid sinus to fundus (S-F line) of the internal acoustic meatus (IAM) the location of the bony labyrinth and angles of approach as well as the distances from the inferolateteral internal acoustic meatus (ILIAM) to fundus, common crus and vestibule were measured in milimeters. The location of the juguler bulb was also assessed. At the second step the cadaveric specimens were approached via the suboccipital-retrosigmoid route in an affort compare the measured values of CT with that of the microsurgical ones. Results: The location of the bony labyrinth on CT was found to be lateral, on and medial to S-F line in %40 (n:8), %35 (n.7) and %25 (n:5) of the respectively. In two (%10) of the specimens a high located jugular bulb was noted. When the measured values from the CT scanning were compared with the distance that were obtained via microsurgical measurements of the distances of ILIAM-fundus, ILIAM-common crus and ILIAM-vestibule no statisticaly significance was observed (p>0.05). In four (%20) specimens in which the bony labyrinth was located medial to the S-F line, visualization of the fundus required a wider angle of approach. Conclusion: During planing of the surgical approaches that involve the IAM, preoperative bone window CT scanning of the temporal bone provides relevant to information in order to minimize morbidity.
