Nöroşirürjide İntraoperatif Manyetik Rezonans Görüntüleme (İo-MRG): Avantaj ve Dezavantajları

Özet
Nörolojik morbiditeyi artırmadan patolojik lezyonların total rezeksiyonun sağlanması için intraoperatif rezeksiyon kontrolü giderekdaha önemli hale gelmiştir. İntraoperatif manyetik rezonans görüntüleme (İo-MRG) patolojik lezyon sınırlarını intraoperatif tanımlamayave cerrahın oryantasyonunu düzeltmeye yardımcı olabilir. İo-MRG kullanarak bir kranyal nöroşirürjikal girişim 1995 yılında Boston’daDr. Black ve ekibi tarafından yapıldı. Ameliyat sırasında beyin şifti ile cerrahi rezeksiyonun derecesini tayin etme ve cerrahi sırasındakikanama, infarkt ve ödem gibi komplikasyonları değerlendirebilme imkanı gibi birçok faktör İo-MRG’nin doğuşunda etkili olmuştur.Başlıca iki farklı İo-MRG tasarımı vardır: Boston ve Heidelberg tasarımları. Boston tasarımında tüm cerrahi MRG cihazı içindeyapılır. Heidelberg tasarımında ise ameliyathane ile görüntüleme odası birbirinden ayrılmıştır. GE, Siemens, Phillips, Medtronic veHitachi İo-MRG’nin endüstri liderleridir. Piyasaya yeni giren firmalar için yüksek teknik engeller vardır. Başlangıcından bu yana İo-MRG hem çocuklarda hem de erişkinlerde sadece nöroonkolojik cerrahide değil aynı zamanda nörovasküler, spinal, stereotaktik,ve fonksiyonel cerrahide de başarıyla kullanılmıştır. Ancak İo-MRG halen pahalı bir yöntemdir ve ameliyathanede ekstra güvenlikkoşullarını gerektirir. Bununla beraber hızla ilerleyen teknolojiyle kullanımının yaygınlaşması beklenebilir.
Abstract
Intraoperative resection control has become important for total resection of lesions without increasing neurological morbidity. Intraoperative MRI (Io-MRI) may help to identify the borders of pathologic lesions intraoperatively and correct the surgeon’s orientation. A cranial surgery using Io-MRI was first performed in Boston in 1995 by Black and his team. Many factors such as the possibility of assessment of the brain shift and the degree of surgical resection, as well as evaluating the possibility of complications such as bleeding, infarction and edema during surgery were effective in the emergence of Io-MRI. There are two main Io-MRI designs: Boston and Heidelberg. In the design of Boston, all of the surgery is performed in the MRI device. In the Heidelberg design, the operating and imaging rooms are separated. GE, Siemens, Phillips, Medtronic and Hitachi are leaders of the Io-MRI industry. There are high technical barriers for new entrants in the industry. Io-MRI has been used successfully in both children and adults, not only in neuro-oncological surgery but also in neurovascular, spinal, stereotactic and functional surgery. Although Io-MRI is still expensive and requires extra safety conditions in the operating room, its use can be expected to become widespread with the rapidly advancing technology.
