Özet

Pineal bölgeye yönelik cerrahi girişimler; derin yerleşim, karmaşık anatomik komşuluklar ve vasküler yapılar nedeniyle zorlayıcı olabilmektedir. Bu bölge; üçüncü ventrikülün posterior kovuğu, velum interpositum sisterni ve kuadrigeminal sistern tarafından sınırlandırılır. İçerisinde pineal bez, posterior kommisür, habenüler kommisür, tektal plağın superior ve inferior kollikülü bulunur. Ayrıca interserebellar venler, Galen veni, presantral serebellar ven ve medial posterior koroidal arter gibi önemli vasküler yapıları barındırır. Pineal tümörlere cerrahi yaklaşımda başlıca supraserebellar infratentoryal ve oksipital transtentoryal cerrahiler olmak üzere; tümörün yerleşim yeri, uzanımı ve boyutlarına göre pek çok farklı yaklaşım tanımlanmıştır. Cerrahi planlamada ve komplikasyonların önlenmesinde; pineal bölgeyi oluşturan anatomik yapıların tanınması ve varyasyonların bilinmesi önem taşımaktadır.

Abstract

Pineal region surgery is challenging because of the deep location, complex anatomical relations and the vascular structures. Theborders of the pineal region consist of the third ventricle posterior extension, the velum interpositum cistern and the quadrigeminalcistern. The region contains the pineal gland, posterior commissure, habenular commissure, and the superior and inferior colliculi ofthe tectal plate. It also contains important vascular structures such as intercerebellar veins, the great vein of Galen, the precentralcerebellar vein and the medial posterior choroidal artery. There are many surgical approaches for pineal tumors depending on thetumor location, extension or size. Supracerebellar infratentorial and occipital transtentorial approaches are most commonly preferred.It is important to know the anatomical structures and their variations for surgical planning and the prevention of complications.
