Özet

Beyin cerrahisindeki en kompleks girişimlerden biri olan presigmoid yaklaşım da amaç kısmi ya da tam petrozektomi sonrası presigmoid bölgeden posterior kranial fossaya ulaşmaktır. Presigmoid yaklaşımlar lateral suboksipital yaklaşımların anterior uzantısıdır. Esasen ventral beyin sapı ve klivus bölge patolojilerine yaklaşım için tanımlanmış olan presigmoid yaklaşımlar ve modifikasyonları sayesinde transpetrozal yolla özellikle serebellopontin köşe ve ponsu mükemmel bir şekilde ortaya koymak, ayrıca posterior sirkülasyonun bazı ulaşımı zor anevrizmalarına ulaşmak mümkündür. Makalede güncel literatür eşliğinde presigmoid yaklaşımın, her biri petrozektominin ilerleyen basamaklarından oluşan 3 temel varyasyonu: Retrolabirintin yaklaşım, translabirintin yaklaşım ve transkoklear yaklaşımın anatomik temellere dayalı cerrahi teknikleri, endikasyonları ve komplikasyonları tartışılacaktır.

Abstract

The aim of presigmoid approaches, one of the most complicated approaches in neurosurgery, is reaching the posterior cranialfossa after partial or total petrosectomy through the presigmoid area. Presigmoid approaches are anterior extensions of the lateral suboccipital approaches. It is possible to particularly expose the cerebellopontine angle and pons and also to reach some of theposterior circulation aneurysms that are hard to reach via presigmoid approaches and its modifications through the transpetrosalroute, which were essentially defined to reach the pathologies of the ventral brainstem and clival region. In this article, the anatomical basis of the surgical techniques, indications and complications of the retrolabyrinthine approach, translabyrinthine approach and transcochlear approach, the 3 variations of the presigmoid approach that each are progressive steps of petrosectomy, will be discussed in the light of the current literature.
