Özet

Santral kord sendromu inkomplet omurilik yaralanmasının en sık görülen tipidir. Genellikle travma ile servikal hiperekstansiyonsonrası, özellikle yaşlı hastalarda daralmış olan servikal spinal kanalda omurilik yaralanması sonucu ortaya çıkabilir. Santral kordsendromu, üst ekstremitelerde alt ekstremiteye oranla daha ağır motor defisit, yaralanma seviyesinin altında değişen oranlardaduyusal kayıp ve mesane disfonksiyonu ile kendini gösterir. Teknolojik imkânların ve cerrahi tekniklerin sınırlı olduğu eski yıllardaanlaşılabilir şekilde konservatif tedavi yöntemleri ön planda tutuluyordu. Son yıllardaki çalışmalar, yüksek hızlı dril, ultrasonik kemikkesici ile mikroskop gibi teknolojik gelişmeler ve artan cerrahi tecrübeler sayesinde cerrahinin sonuçları daha iyi hâle gelmiştir.Günümüzde instabilitesi olmayan, omurilik basısı olmayan ve hafif nörolojik defisitli (ASIA-D) hastalara konservatif tedavi önerilir.İnstabilitesi olan, omurilik basısı olan, hastalara ise cerrahi tedavi önerilmektedir. Cerrahi tedavi genellikle ilk 24 saatte yapılmasındayarar vardır.

Abstract

Central cord syndrome is the most common type of incomplete spinal cord injury. It usually occurs after trauma with cervical hyperextension, especially as a result of spinal cord injury in the cervical spinal canal, which has narrowed in elderly patients. The central cord syndrome is manifested by severe motor deficits in the upper extremities compared to the lower extremities, sensory loss at varying levels below the level of injury, and bladder dysfunction. In the old years, when technological possibilities and surgical techniques were limited, conservative treatment methods were prioritized. In recent years, the results of surgery have become better thanks to technological developments such as high speed drills, ultrasonic bone cutter and microscope and increased surgical experience. Today, conservative treatment is recommended for patients with instability, spinal cord compression, and mild neurological deficit (ASIA-D). Surgical treatment is recommended for patients with instability and spinal compression. Surgical treatment is generally useful in the first 24 hours
