Antidepresan tedavisinin depresyon ve yorgunluğu olan MS'li hastalarda yorgunluk üzerine etkisi

Özet
Bilimsel zemin: Yorgunluk ve depresyon Multipl Sklerozda(MS) sık görülen semptomlardır. Yorgunluk olguların yaklaşık %53-90'ında majör semptom olarak bildirilmektedir. MS'li olguların tüm yaşamları boyunca depresyonla karşılaşma olasılığı %50 olduğu saptanmıştır. Amaç: MS'de yorgunluk ile depresyon arasında pozitif bir ilişki varlığını ve depresyonun yorgunluk şiddetini arttırdığını bildiren çalışmalardan hareket ile depresyonu ve yorgunluğu olan olgularda antidepresan tedavinin yorgunluk üzerine etkisini araştırmak.Gereç ve Yöntemler: Çalışmaya son altı ay içinde atak geçirmemiş ve kortizon tedavisi almamış kesin rela'psing-remitting MS (RRMS) tanılı 36 olgu alındı. Muayene ile depresyon ve yorgunluğu olduğu saptanan olgular psikiyatri uzmanı tarafından değerlendirilerek depresyon tanısı doğrulandı. Antidepresan tedavi başlanan olgular 6 ay süre ile izlendi. Tedavinin etkinliği; O(tedayinin başlangıcı), 3.ve 6. ay sonunda uygulanan Hamilton depresyon değerlendirme ölçeği (HAM-D) ve yorgunluk değerlendirme ölçeği(YDÖ) ile hesaplandı. Bulgular: 3 ve 6. ay sonunda depresyon ve yorgunluk puanlarındaki düşme istatistiksel olarak anlamlı (p0.05). izlenim: Antidepresan tedavi ile yorgunlukta kısmi bir düzelme sağlanabilir.
Abstract
Effects of Antidepressant Treatment on Fatigue in MS Patients with Depression and Fatigue Scientific background: Fatigue and depression are frequently observed symptoms in Multiple Sclerosis (MS). Fatigue has been reported as the major symptom in 53-90% of MS patients. Lifetime incidence of depression has been'found as 50% in MS cases. Objectives: There are several studies reporting a positive correlation between depression and fatigue in MS. It has been reported that depression increases the severity of fatigue in these patients. The objective of this study is to evaluate the effects of antidepressant treatment in MS patients having depression and fatigue. Material and methods: 36 cases with definite relapsing-remitting MS (RRMS) who had been free of attacks and steroids for six months were included in this study. Patients, whose examination revealed depression and fatigue, were diagnosed with depression by a psychiatrist. Cases were administered antidepressants and followed by 6 months. The efficacy of the treatment was calculated by Hamilton Depression Rating Scale (HAM-D) and Fatigue Evaluation Scale administered at baseline and at the end of 3rd and 6th months. Findings: The decrease in depression and fatigue scores at the end of 3rd and 6th months were found to be statistically significant. (p0.05). Conclusion: A partial improvement can be obtained in fatigue by antidepressant treatment.
