Özet

Bilimsel zemin: Peroksinitrit (PRN) güçlü bir oksidan olup doku ve hücrelere zarar verir ve multipl sklerozun (MS) patogenezinde major bir rol oynar. Ürik asit (ÜA) insanlarda pürin metabolizmasının doğal olarak oluşan son ürünüdür ve serumda en fazla bulunan endojen antioksidandır. Ürik asit, PRN'ye seçici olarak bağlanıp onu inaktive eder. Yakın dönemde yapılan çalışmalarda MS hastalarında serum ÜA seviyesinin düşük olabileceği öne sürülmektedir. Amaç: Bu çalışmanın amacı, endojen antioksidan savunmanın, MS'nin patogenezindeki rolünü araştırmak için, multipl skleroz hastalarında serum ürik asit düzeylerini belirleyerek, demografik ve klinik özellikler ile olan ilişkisini incelemektir. Gereç ve yöntemler: Ataksız dönemde bulunan 206 MS hastasında (151 kadın) ve 38 yaş ve cins eşleştirilmiş sağlıklı bireyden oluşan kontrol grubunda, serum ÜA değerleri belirlendi. Klinik alt tiplerine göre 206 hastanın; 159'u relapsing remitting (RR-MS), 37'si sekonder progresif (SP-MS) ve 10 hasta da primer progresif (PP-MS) formunda idi. Hastaların dizabiliteleri EDSS (Expanded Disability Status Scale) skoru ile değerlendirildi. Hastalar 12 ay boyunca takip edildi ve bu süre içinde 206 hastanın 45'inde atak gözlendi. Atak gözlenen hastalarda tedaviden hemen önce serum ÜA seviyeleri ve EDSS değerleri belirlenerek ataksız dönem ile karşılaştırıldı. Abstract

The Relationship Between the Multiple Sclerosis and Endogenous Antioxidan Serum Uric Acid Scientific background: Peroxynitrite (PRN) is a strong oxidant capable of damaging tissues and cells, and plays a major role in the pathogenesis of multiple sclerosis (MS). Uric acid (UA) is the naturally occuring final product of the purine metabolism in humans and the most prominent endojen antioxidant in serum. Uric acid selectively binds and inactivates the PRN. Recent studies suggested that patients with multiple sclerosis (MS) have lower levels of serum UA. Objective: To investigate the role of the endogenous antioxidant defence in MS pathogenesis, by determining the serum UA levels of the MS patients and, examining the relationships with demographic and clinical characteristics of the patients. Materials and methods: Serum uric acid levels were determined in 206 MS patients (151 women) all in the remission period, and in 38 age and sex matched voluntary healthy subjects, served as a control group. Of the 206 patients; 159 had the relapsing-remitting (RR) type of the disease, 37 secondary progressive (SP) and 10 primary progressive (PP).
