Özet
Beyin tümörleri çocukluk çağı tümörleri arasında ilk sıralarda yer almaktadır. Erişkin yaş grubuna göre posterior fossa yerleşimi daha büyük oranlardadır (%30-56). Düşük grade'li astrositomalar, medulloblastomalar ve epandimomalara sık, dermoid tümörler, rabdoid tümörler, teratoma, yüksek grade'li glial tümörlere, meningioma ve metastazlara daha nadir rastlanmaktadır. 1988-2000 yılları arasında İstanbul Üniversitesi Cerrahapaşa Tıp Fakültesi Nöroşirurji ve Bakırköy Ruh ve Sinir Hastalıkları Hastanesi II. Nöroşirurji kliniklerinde 73 çocuk olgu posterior fossa tümörü nedeni ile ameliyat edildi. 37 olguda (%51) operasyon öncesinde eksternalventriküler drenaj yolu ile beyin-omurilik sıvısı boşaltıldı. 48 olgu (%66) yüzükoyun, 22 olgu (%30) oturur, 3 olgu (%4) yan yatar pozisyonda opere edildiler. 30 olgu medulloblastoma (%41), 28 olgu düşük gradeli astrositoma (%38), 11 olgu epandimoma (%15), 2 olgu atipik teratoid rabdoid tümör (%3), 2 olgu metastaz (%3) tanısı aldı. Eksternal ventriküler drenaj yolu ile beyin-omurilik sıvısı boşaltılan 37 olgudan 10 olguya (%27) ve bu işlem uygulanmayan 36 olgudan 9 olguya (%25) ventrikülo-peritoneal şant takıldı. Operasyon sonrasında düşük gradeli astrositomalara ek tedavi uygulanmazken izlemde kalan diğer olgu grubuna radyoterapi ve/veya kemoterapi uygulandı. Sunumuzda posterior fossa tümörü nedeni ile opere edilen 73 olgu incelenerek sonuçlar literatür bilgileri eşliğinde tartışılmıştır.
Abstract
Abstract: Brain tumors take an important place among the most frequent malignancies of children. Compared with the adult age group, posterior fossa tumors are seen in higher percentage in children (30-56%). Low grade astrocytomas, medulloblastomas and epandymomas are seen frequently, dermoid tumors, rhabdoid tumors, teratomas, high grade gliomas are seen occasionaly, meningiomas, metastasis are seen rarely in this age group. 73 child cases with posterior fossa tumor were operated in Neurosurgery Departments of Cerrahpaşa Medical School and Bakırköy Psychiatry and Neurological Science State Hospital between 1988-2000. In 37 cases (51%) cerebro-spinal fluid was drained with external ventricular drainage preoperatively. 48 cases (66%) were operated in prone, 22 cases (30%) in sitting, 3 cases (4%) in lateral decubitis position. The results of 30 (41 %) cases were medulloblastoma, 28 (38%) cases were low grade astrocytoma, 11 (15%) cases were epandymoma, 2 (3%) cases were theratoid-rhabdoid tumor, 2 (3%) cases were metastasis. Ventriculo-peritoneal shunt was applied to 10 (27%) cases of 37 cases whose cerebro-spinal fluid drained via external ventricular drainage method; again ventriculo-peritoneal shunt was applied to 9 (25%) cases of 36 cases whose cerebro-spinal fluid was not drained. For low grade astrocytoma cases no adjuvant therapy was applied postoperatively, for the rest of the cases adjuvant radyotherapy and/or chemotherapy was applied. Our results were reviewed in association with the literature.
