Klinik Örneklerde Mycobacterium tuberculosis Saptanmasında İki Gerçek Zamanlı PCR Sisteminin Tanısal Performansının Karşılaştırılması§

Özet

Amaç: Erciyes Üniversitesi Tıp Fakültesi Mikobakteriyoloji Laboratuvarına kabul edilen klinik örneklerde Mycobacterium tuberculosis suşlarını belirlemek için iki gerçek zamanlı PCR sistemlerinin tanısal performansının karşılaştırılması amaçlan- mıştır. Gereç ve Yöntem: Mikobakteriyoloji Laboratuvarına tüberküloz tanısı amacıyla gönderilen toplam 50 klinik örnek çalışmaya alın- dı. Kültür pozitif örneklerin 25i M. tuberculosis kompleks (MTBC) grubu ve 7si tüberküloz dışı mikobakteri (TDM) olarak tanımlan- dı. BACTEC MGIT 960 sistem ile duyarlılık testleri çalışıldı. Toplam 50 klinik örneğin moleküler tanısı ise, Mycobacterium tuberculosisi belirleyen gerçek zamanlı PCR; artus® M. tuberculosis RG PCR Kit (Qiagen, Hilden, Almanya) (artus® MTB-PCR) ve GeneXpert MTB/RIF (Cepheid, Sunnyvale, CA, ABD) sistemleri ile yapıldı. Bulgular: Her iki sistemde de klinik örneğin; %58inde MTBC pozitif ve %42sinde negatif olarak bulundu. Kültür yönteminin altın standart kabul edildiği bu çalışmada, artus® MTB-PCR ve GeneXpert MTB/RIF testlerinin sırasıyla duyarlılığı %72 ve %96, özgüllüğü %56 ve %80, pozitif prediktif değeri (PPD) %62 ve %82 ve negatif prediktif değeri (NPD) %66 ve %95 olarak saptandı. Abstract

Objective: This study is aimed to compare the diagnostic perfor- mance of two real-time polymerase chain reaction (PCR) systems for detecting Mycobacterium tuberculosis in clinical samples. Materials and Methods: Total of 50 clinical specimens sent for the diagnosis of tuberculosis to Mycobacteriology Laboratory of Erciyes University, Medical Faculty, were included in the study. Twenty-five of the culture positive samples were defined as Mycobacterium tuberculosis complex (MTBC) and 7 as non- tuberculous mycobacteria (TDM). Susceptibility tests were perfor- med with BACTEC MGIT 960 system. Mycobacterium tuberculosis real-time PCR; artus® M. tuberculosis RG PCR Kit (Qiagen, Hilden, Germany) (artus® RTUS MTB-PCR and GeneXpert MTB/ RIF (Cepheid, Sunnyvale, CA, USA) assay systems were used for the molecular diagnosis of the 50 clinical samples. Results: The results of the both of the molecular diagnostic systems tested revealed that 58% of the clinical samples were MTBC positive, and 42% were negative. When culture was consi- dered as the gold standard; sensitivity, specificity, positive predic- tive value (PPV) and negative predictive value (NPV) of artus® MTB-PCR and GeneXpert MTB/RIF were found to be 72% vs 96%, 56% vs 80%, 62% vs 82%, 66% vs 95%, respectively.
