Özet

Bu araştırma, HBeAg pozitif kronik hepatit B'li olgularımızın biyokimyasal, serolojik profili ve klinik gidişlerinin değerlendirilmesi amacıyla, Ocak 2003-Aralık 2004 tarihleri arasında yürütülmüştür. Çalışmaya alınan 12 olgunun dokuzu (% 75) erkekti ve ortalama yaşları 31 (yaş aralığı 15-52) idi. Dokuz olgunun ALT seviyeleri ortalama 30 U/L bulunurken üç olguda normal seviyelerin üzerindeydi (76,105,237 U/L). Yedi olguda HBV DNA Hibride Capture yöntemi ile çalışılabilmiştir. HBV DNA, olgularımızın dördünde pozitif (% 57), üç olguda ise negatif olarak saptanmıştır (% 43). Olguların hiçbirinde anti- HCV, anti-HDV ve anti-HIV pozitifliği saptanmamıştır. İzlem süresince HBeAg/anti-HBe serokonversiyonu bir olguda (% 8.3) takibin 39. ayında gelişmiştir. Ultrasonografide hiçbir olguda karaciğerde yer kaplayan oluşuma rastlanmamıştır. HBeAg pozitif kronik hepatit B infeksiyonu daha çok vertikal geçiş sonrası geliştiği düşünülen, uzun süreli takipte serokonversiyon oranı yüksek, komplikasyonların sık gelişmediği bir infeksiyondur. Bu hastalık grubunda yıllık takiplerin yeterli olduğu ancak karaciğer enzim yüksekliği olan dönemlerde daha yakın takibin gerekebileceği sonucuna varılmıştır

Abstract

The aim of the study was to evaluate patients with the HBeAg positive chronic hepatitis B as biochemical, serological and clinically between Januay 2003-December 2004. Twelve patients ( 9 male, mean age 31) were enrolled in the study. Mean ALT levels of nine patients were 30 U/L but it was elevated in three patients (76,105,237 U/L). HBV DNA (by hibridization) was studied in seven patients and it was positive in four (% 57) and negative in three (43 %) ones. Anti-HCV, anti-HDV and anti- HIV were negative in all patients. During following period HBeAg/anti-HBe seroconversion developed in one (8.3 %) patient after 39 months. There was not any space occupying lession in ultrasonography. In conclusion; in major of chronic hepatitis B patients transmission was thought to be vertical. In long term following period, in this group, rate of seroconversion is high and complication ratio is low. Twelve months intervals for monitoring of this group is sufficient but while the liver function test is high the following period should be closer.
