Nozokomiyal Kökenli Pseudomonas aeruginosa İzolatlarında Metallo-Beta-Laktamaz Sıklığı

Özet
Amaç: Bu çalışmada nozokomiyal infeksiyon etkeni olarak izole edilen P. aeruginosa izolatlarında Metallo-BetaLaktamaz (MBL) üretimi ile bu bakterilerin antibiyotiklere duyarlılıklarının belirlenmesi amaçlandı.Gereç ve Yöntem: Laboratuvarımızda izole edilen ve CDC kriterlerine göre hastane infeksiyonu etkeni olarak tanımlanan, imipenem dirençli 62 P. aeruginosa izolatı çalışmanın gerecini oluşturdu. MBL varlığını belirlemek için kombine disk testi uygulandı. Çalışmaya alınan imipenem dirençli P. aeruginosa izolatlarının antibiyotik duyarlılık testlerinde BD Phoenix otomatize mikrobiyoloji sisteminden (Becton Dickinson, ABD) yararlanıldı.Bulgular: Hastane infeksiyonu etkeni imipenem dirençli P. aeruginosa izolatlarının kombine disk testi ile 53 (%85)'ünde MBL varlığı belirlendi. İmipenem dirençli izolatların diğer antibiyotiklere direnç oranları; amikasin %10, siprofloksasin %53, seftazidim %65, levofloksasin %68, aztreonam %84, gentamisin %90, piperasilintazobaktam %92, sefepim %97, piperasilin %100 olarak bulundu.Sonuç: Nozokomiyal kökenli P. aeruginosa suşlarında MBL üretimi giderek artmaktadır. Dirençten sorumlu olan bu enzim, her laboratuvarda uygun bir fenotipik yöntemle rutin olarak saptanmalı ve rapor edilmelidir. Bu uygulama direnç yayılımının önlenmesinde ve infeksiyonların tedavisinde önemli yararlar sağlayacaktır
Abstract
Objective: The aim of this study was to investigate the frequency of metallo-beta-lactamases (MBL) and antibiotic susceptibility profiles of nosocomial P. aeruginosa strains. Material and Methods: A total of 62 imipenem resistant P. aeruginosa isolates identified as hospital-acquired according to the CDC criteria, were included in the study. The combined disc test was performed to determine the presence of MBL. Phoenix automated antibiotic susceptibility testing system (Becton Dickinson, USA) was used for the antibiotic susceptibility testing of imipenem-resistant P. aeruginosa isolates included in the study. Results: The presence of MBL was detected by combined disc test in 53 (85%) of the imipenem resistant nosocomial P. aeruginosa isolates. The rates of resistance to the other tested antibiotics among the imipenem resistant isolates were found to be: amikacin 10%, ciprofloxacin 53%, ceftazidime 65%, levofloxacin 68%, aztreonam 84%, gentamicin 90%, piperacillin-tazobactam 92%, cefepime 97% and piperacillin 100%. Conclusion: MBL production gradually increases among the nosocomial P. aeruginosa isolates. This enzyme which is responsible for resistance to beta-lactam agents, including the carbapenems, should routinely be tested in every laboratory by appropriate phenotypical methods and results should be reported. This practice will be of great value in preventing the spread of resistant isolates and for treatment success.
