Çeşitli Klinik Örneklerden İzole Edilen Escherichia coli ve Klebsiella pneumoniae İzolatlarının Antibiyotiklere Direnç Durumu

Özet
Amaç: Genişlemiş spektrumlu beta laktamazlar (GSBL)Escherichia coli ve Klebsiella pneumoniae başta olmak üzereEnterobacteriaceae'da sık görülmektedir. Beraberinde florokinolonve aminoglikozid direnci de görülebildiğinden gelişenenfeksiyonların tedavisi daha da güçleşmektedir. Bu nedenleuygun empirik tedavide direnç durumunun bilinmesi son dereceönemlidir. Bu çalışmada hastanemizde yatan hastalarınçeşitli klinik örneklerinden izole edilen E. coli ve K. pneumoniaeizolatlarının GSBL üretme oranını ve antibiyotik duyarlılıkdurumunu belirlemeyi amaçladık.Gereç ve Yöntem: Çalışmaya Ocak-Aralık 2012 tarihleri arasındaLütfi Kırdar Kartal Eğitim ve Araştırma Hastanesi'ndeyatan hastaların klinik örneklerinde ilk üreyen 385 E. coli ve154 K. pneumoniae izolatı dâhil edildi. Aynı hastaların farklıkültürlerinden üreyen izolatlar çalışma dışı bırakıldı. Üreyenbakterilerin tanımlanması, antibiyotik duyarlılıkları ve GSBLvarlığı VITEK2 otomatize sistemiyle (bioMérieux, Fransa)belirlendi.Bulgular: Çalışmamızda 385 adet E. coli ve 154 adetK. pneumoniae izole edildi. Üç yüz seksen beş E. coli izolatının212'sinin (%55), 154 K. pneumoniae izolatının 86'sının(%55.8) GSBL ürettiği saptandı. GSBL pozitif E. coli'de siprofloksasine%70.7, kotrimoksazole %74.0 amikasine %16.5,gentamisine %43 oranında direnç saptandı. GSBL pozitifK. pneumoniae'da ise siprofloksasine %66.2, kotrimoksazole%68.6, amikasine %16.2, gentamisine %43.0 oranında dirençbelirlendi. GSBL üreten E. coli'lerin %1.4'ü imipeneme, %2.8'iertapeneme ve %0.9'u meropeneme dirençli idi. GSBL pozitifK. pneumoniae'da %10.5 imipeneme, %7.0 meropeneme, %11.6ertapeneme direnç bulundu. Kolistine dirençli suş tespit edilmedi.Sonuç: GSBL pozitif E. coli ve K. pneumoniae'da direnç oranıen düşük olan antibiyotiklerin kolistin ve karbapenemler oldu-ğu görüld.
Abstract
Objective: Extended-spectrum beta-lactamases (ESBL) arefrequently seen in Enterobacteriaceae species, especiallyEscherichia coli and Klebsiella pneumoniae. Treatment ofinfections caused by these bacteria is becoming even morechallenging due to simultaneous co-resistance to aminoglycosidesand fluoroquinolones. Thus, knowledge about thestate of resistance is extremely important for appropriateempirical antibiotic treatment. This study was conducted todetermine the antimicrobial susceptibility patterns and therates of ESBL production in E. coli and K. pneumoniae strainsisolated from the clinical specimens of hospitalized patientsin our hospital.Materials and Methods: A total of firstly grown isolates of385 E. coli and 154 K. pneumoniae on the bacterial cultures ofclinical specimens of consecutive patients hospitalized betweenJanuary and December 2012 were analyzed, Strains isolatedsimultaneously from the other body sites of the same patientswere excluded from the analysis. VITEK 2 automated system(bioMérieux, France) was used for bacterial identification,susceptibility testing, and detection of the presence of ESBL.Results: In our study 212 (55%)out of 385 E. coli, and 86(55.8%) out of 154 K. pneumoniae strains were found toproduce ESBL. The resistance rates of ESBL-producing E. coliwere 70.7% for ciprofloxacin, 74.0% for co-trimoxazole,16.5% for amikacin, and 43.0% for gentamicin. The correspondingrates for ESBL-producing K. pneumoniae were 66.2,68.6, 16.2, and 43%, respectively. Of ESBL-producing E. colistrains, 1.4% were resistant to imipenem, 2.8% to ertapenem,and 0.9% to meropenem. Among ESBL-producing K. pneumoniaestrains, 10.51, 7, and 11.6% of isolates exhibited resistance toimipenem, meropenem, and ertapenem, respectively. Any resistantstrain to colistin was not detected.Conclusion: Among ESBL-producing E. coli and K. pneumoniaestrains the lowest rates of resistance was seen against colistinand carbapene.
