Özet
Sağlık sisteminde kalite uygulamalarının amacı hasta güvenliğidir. Laboratuvarlar bu süreçte tanının doğru konması ile hastaya doğru zamanda doğru tedavinin uygulanmasına aracılık eder. Sağlık sistemlerinde süreçler birbirinden bağımsız olmayıp, her süreç bir diğerini etkileyebilmektedir. Bu olgu sunumunda, laboratuvarda yanlış hastadan alınan kan örneğinin fark edilmesi ve kök-neden analizi yapılarak benzer hataların oluşmasına yönelik önlemlerin alınması vurgulanmıştır.
Abstract
Patient safety lies at the core of quality applications in health systems. Laboratory results mediates the process of making accurate diagnosis, and timely application of effective treatment. The processes in the health care system are not independent from each other, and each process can affect the other. In this care report discernment of an erroneously labeled blood samples, and conduction of rootcause analysis so as to implement the necessary measures to prevent the reccurrence of the same mistakes have been emphasized.
