Klebsiella İzolatlarının Antimikrobiyal Direnç Oranlarının Değerlendirilmesi

Özet

Amaç: Bu çalışmada, hastanemiz mikrobiyoloji laboratuvarına gönderilen çeşitli klinik örneklerinden soyutlanan Klebsiella izolatlarının antibiyotiklere direnç durumlarının belirlenmesi amaçlanmıştır. Gereç ve Yöntem: İzole edilen Klebsiella cinsi bakterilerin çeşitli antibiyotiklere direnç oranları retrospektif olarak incelenmiştir. Çalışmaya, 2012-2014 yılları arasında, çeşitli klinik örneklerde üreyen ve etken olarak kabul edilen 507 adet Klebsiella cinsi bakteri alınmıştır. İzole edilen mikroorganizmaların tanımlanmaları ve antibiyotik duyarlılık testleri VITEK 2 ID-AST (bioMérieux, Fransa) otomatize sistemi ile yapılmıştır ve sonuçlar "Clinical and Laboratory Standards Institute" standartlarına göre yorumlanmıştır. Bulgular: Çalışmaya dâhil edilen 507 Klebsiella izolatının 479'unun (%94.5) Klebsiella pneumoniae, 28'inin (%5.5) Klebsiella oxytoca olduğu saptanmıştır. Elde edilen izolatlar; sıklık sırasıyla 208'i (%41) idrar, 121'i (%23.9) kan, 83'ü (%16.4) trakeal aspirat, 76'sı (%15) yara ve 19'u (%3.7) diğer klinik örneklerinden izole edilmiştir. Beş yüz yedi adet Klebsiella izolatların 330'unda (%65.1) GSBL üretimi saptanmıştır. GSBL üreten ve üretmeyen izolatlarda in-vitro en etkili antimikrobiyaller her iki tür için amikasin ve imipenem olarak bulunmuştur. GSBL üreten K. pneumoniae izolatlarında amikasin direnci %18 oranında, imipenem direnci ise %23.5 oranında saptanmıştır.Sonuç: Tüm merkezler kendi etken ve direnç profilini saptayarak uygun antibiyotik politikalarını oluşturmalı ve profilaktik tedaviler için yol gösterici olan güncel bilgileri takip etmelidirler

Abstract

Objective: The aim of this study was to determine the antimicrobial resistance rates of Klebsiella strains which were isolated from various clinical samples sent to the medical microbiology laboratory of our hospital. Material and Method: The resistance rates of Klebsiella strains to various antibiotics were investigated, retrospectively. The study included 507 Klebsiella strains that were isolated from various clinical samples and accepted as pathogens during 2012-2014 years. The identification and antibiotic susceptibility tests of the isolated bacteria were performed by VITEC 2 ID-AST (bioMérieux, France) and the results were evaluated according to the standards of Clinical and Laboratory Standards Institute. Results: In this study, 479 (94.5%) of the Klebsiella isolates were identified as Klebsiella pneumoniae while 28 (5.5%) were identified as Klebsiella oxytoca. The strains were isolated from (n=208; 41%) from urine, blood (n=121; 23.9%) tracheal aspirate (n=83; 16.4%), wound sites (n=76; 15%) and other sources (n=19; 3.7%). Of the 507 Klebsiella isolates 330 (65.1%) were detected as ESBLpositive. Amikacin and imipenem were found to be the most effective antimicrobials in vitro for ESBL producing and nonproducing isolates. Amikacin, and imipenem resistance rates were 18% and 23.5%, respectively in ESBL-producing K. pneumoniae isolates. Conclusion: All centers should identify their causative infectious agents, and and antibiotic resistance profiles, and formulate their antibiotherapy protocols, and investigate guiding current information.
