Konya İlinde Klinik Örneklerden İzole Edilen Mycobacterium tuberculosis Kompleks Suşlarının Birinci Seçenek Anti-tüberküloz İlaçlara Direnç Oranları

Özet

Amaç: Bu çalışmada, 2014 ve 2015 yılını içeren iki yıllık süreçte, laboratuvarımızda izole edilen Mycobacterium tuberculosis kompleks izolatlarının birinci seçenek antitüberküloz ilaçlara duyarlılıklarının retrospektif olarak araştırılması amaçlanmıştır.Gereç ve Yöntem: Tüberküloz ön tanısı ile gönderilen şüpheli örnekler Ehrlich-Ziehl-Neelsen yöntemi ile boyanarak, mikroskobik olarak değerlendirilmiş, Löwenstein-Jensen besiyeri ve otomatize kültür sistemine (Mycolor TK, Salubris A.Ş., İstanbul) ekimleri yapılmıştır. Üreme görülen kültür tüplerinde Mycobacterium tuberculosis kompleks tanımlaması ve streptomisin, izoniazid, rifampisin, etambutol duyarlılıkları çalışılmıştır. Bulgular: Çalışmaya laboratuvara gönderilen 5406 klinik örnekten, Mycobacterium tuberculosis kompleks izole edilmiş 266 suş dâhil edilmiştir. Her hasta için tek bir suş çalışmaya alınmıştır. Streptomisin, izoniazid, rifampisin ve etambutole toplam direnç oranları sırasıyla %4.5, %6.4, %9 ve %4.5 olarak bulunmuştur. Hastaların %83.1'i tüm antibiyotiklere duyarlı olup, antimikobakteriyel ilaçlar içinde en yüksek direnç rifampisin için saptanmıştır. Çok ilaca direnç oranı ise %2.6 olarak bulunmuştur. Çalışmada ayrıca Ehrlich-ZiehlNeelsen boyama yöntemi ile otomatize kültür sistemi sonuçları karşılaştırmalı olarak değerlendirilmiştir. Sonuç: Tüberkülozda direnç paternlerinin izlenmesi, uygulanacak tedavi protokollerinin belirlenmesinde önemli olduğu gibi uzun vadede direnç gelişiminin önlenmesinde de yararlı olacaktır.

Abstract

Objective: In this study we aimed to retrospectively analyseprimary anti-tuberculosis drug susceptibilities ofMycobacterium tuberculosis complex strains, isolated frompatients with initial diagnosis of tuberculosis during thetwo years period between 2014 and 2015 at ourlaboratory.Material and Methods: Clinical samples obtained frompatients with suspected tuberculosis were stained withEhrlich-Ziehl-Neelsen method and evaluated microscopically.After processing of samples, each sample was inoculatedonto Löwenstein-Jensen medium and automated culturesystem (Mycolor TK, Salubris A.Ş., Istanbul) medium. Ifbacterial growth was detected, Mycobacterium tuberculosiscomplex typing was made and if requested antituberculosisdrug susceptibilities for streptomycin, isoniazid, rifampicinand ethambutol were tested.Results: Out of 5406 clinical samples sent to the laboratory, atotal of 266 Mycobacterium tuberculosis complex iolates wereincluded in the study and only single sample from each patientwas processed. Total resistance rates to streptomycin, isoniazid,rifampicin, and ethambutol were 4.5%, 6.4%, 9% ve 4.5%,respectively. Among antimycobacterial drugs, the highestresistance rate was found for rifampicin and 83.1% of thepatients were susceptible to all of the agents tested. Multidrugresistance was determined as 2.6 percent. In the study,automated culture system was also evaluated in comparisonwith Ehrlich-Ziehl-Neelsen staining method.Conclusion: Monitoring of resistance patterns intuberculosis is important in determining treatment protocolsand will also be useful in preventing the development ofresistance in long term.
