İzmir Tepecik Eğitim ve Araştırma Hastanesi’nde Nazal Sürüntü Örneklerinde Metisilin Duyarlı ve Metisilin Dirençli Staphylococcus aureus Değerlendirilmesi

Özet
Amaç: Hastanemiz mikrobiyoloji laboratuvarına farklı poliklinikve servislerden gönderilen nazal sürüntü örneklerinde üreyenStaphylococcus aureus izolatlarının oranı ve metisilinduyarlılıklarının retrospektif olarak araştırılması amaçlanmıştır.Gereç ve Yöntem: 1 Ocak 2013-31 Aralık 2014 tarihleri arasındaİzmir Tepecik Eğitim Araştırma Hastanesi poliklinik vekliniklerinden Mikrobiyoloji Laboratuvarı’na gönderilen 1373nazal sürüntü örnekleri, %5 koyun kanlı agara ekimleri yapılarak37°C’de 24 saat inkübe edilmiştir. Üreyen bakteri koloni morfolojisiaçısından değerlendirilmiş ve S. aureus olabileceği düşünülenkolonilerden bakteriler konvansiyonel yöntemler ile (Gramboyama, katalaz, koagülaz) tiplendirilmiştir ve pozitif olanlarS. aureus olarak yorumlanmıştır. Bu izolatlar CLSI standartlarınagöre Muller Hinton agar’da Kirby-Bauer disk difüzyon yöntemi ilemetisilin duyarlılığı yönünden incelenmiştir.Bulgular: Değerlendirilen örneklerin 112 (%8.2)’sinde S. aureusüremiş ve bunlardan 101 (%90.2)’i metisilin duyarlı S. aureus(MSSA), 11 (%9.8)’i metisilin dirençli S. aureus (MRSA) olarakbelirlenmiştir. Örneklerin 103 (%92)’ü polikliniklerden, 9 (%8)’uservislerden alınmıştır. Servislerden gelen örneklerden altısındaMRSA saptanmış olup, bunun beşi yoğun bakım hastası ve biri deorgan nakli servisi hastasına aittir.Sonuç: Çalışmamızda, S. aureus nazal taşıyıcılığı %8.2 olarakbulunmuştur. S. aureus hastane kökenli ve toplum kökenlienfeksiyonlar açısından önemli bir patojendir. MSSA’lar poliklinikhastalarında MRSA’lar ise servis hastalarında daha sıklıkla izoleediliyor olsa da servis ve poliklinik hastalarında saptama oranlarıbirbirlerine oldukça yaklaşmaktadır. Yoğun bakım ünitelerindeyatan hastalarda kolonize olan S. aureus suşlarında metisilindirencinin saptanması ve tedavi edilmesi ile oluşabilecek ciddienfeksiyonların önlenmesi sağlanacaktır.
Abstract
Staphylococcus aureus isolates and their methicillin susceptibilities in nasal swab samples sent to the microbiology laboratory of our hospital from different outpatient clinics, and services. Material and Methods: A total of 1373 nasal swab samples sent to the Microbiology Laboratory from Clinics, and Outpatient Clinics of İzmir Tepecik Training and Research Hospital between January 1, 2013 and December 31, 2014 and were incubated for 24 hours at 37°C after inoculation to 5% sheep bloody agar for cultivation. Bacterial growth were evaluated for colony morphology and bacterial colonies suspected to be S. aureus were typed by conventional methods (Gram stain, catalase test, coagulase test) and positive colonies were interpreted as S. aureus. These isolates were tested for susceptibility to methicillin by Kirby-Bauer disc diffusion method on Muller Hinton agar according to CLSI standards. Results: S. aureus was grown in 112 (8.2%) of the evaluated samples, of which 101 (90.2%) were identified as methicillin susceptible S. aureus (MSSA) and 11 (9.8%) as methicillin - resistant S. aureus (MRSA). Hudred and three (92%) samples were obtained from outpatient clinics and 8% (n=9) of them from services. MRSA was detected in six of the samples obtained from the services, five of them belonged to intensive care unit patients and one of them to an organ transplantation unit patient. Conclusion: In our study, nasal, S. aureus carriage was found in 8.2% of the samples. S. aureus is an important pathogen in terms of hospital-acquired and community-acquired infections. Although MSSAs are more prevalent in outpatient clinics and the MRSAs in inpatient clinics, the overall detection rates in both are very close to each other. Identification and treatment of methicillin resistance in S. aureus strains colonized in intensive care units will prevent serious infections that might occur.
