Yaşlı Hastalardaki Enfeksiyonların Geriye Dönük Değerlendirilmesi

Özet
farklı bulgular ile seyredebilmekte ve tanı-tedavide gecikmeyaşanabilmektedir. Bu çalışmada, kliniğimizde izlenen yaşlıhastalardaki enfeksiyon hastalıklarının değerlendirilmesiamaçlanmıştır.Gereç ve Yöntem: Enfeksiyon Hastalıkları ve Klinik MikrobiyolojiKliniği’nde yatan 65 yaş ve üstü hastaların verileri geriye-dönükolarak değerlendirildi. Hastaların demografik özellikleri, alttayatan hastalıkları, yatış süreleri, laboratuvar bulguları, kültürsonuçları ve tanıları kaydedildi.Bulgular: Altmış beş yaş ve üzeri 96 hasta çalışmaya dâhiledildi. Hastaların yaş ortalaması ise 76.5±7 (aralık: 65-98) idi.Altta yatan hastalıklardan hipertansiyon (%23.5), diabetesmellitus (%21), kronik böbrek yetmezliği (%11), kronikobstrüktif akciğer hastalığı (%10) ve kanserler (%9.2) ilksıralardaydı. Hastaneye yatışına neden en sık enfeksiyon ürinersistem enfeksiyonu (%53.5) iken, bunu yumuşak dokuenfeksiyonu (%18.8), pnömoni (%8) ve akut gastroenterit (%5)izlemekteydi. Yatış süreleri ortancası 8,5 (%25-75 aralık:6-12.8) gün idi. Hastaların %73’ünde yüksek ateş belirlendi.Ortalama sedimantasyon hızı 62±33.3 mm/saat, kan lökositsayısı ortanca değeri 10.725/mm3 (%25-75 aralık: 6,413-14,663) ve C-reaktif protein (CRP: 0.05). Ayrıca ürinersistem enfeksiyonu, yumuşak doku enfeksiyonu, pnömoni, akutgastroenterit ve diğer enfeksiyonlar arasında ateş, lökositoz veCRP yüksekliği için anlamlı bir fark bulunmadı (p>0.05).Hastalardan üreyen 38 kültür örneğinin 31’i (%8.6) idrarörneğiydi. İdrar örneklerindeki üremelerin ise 26’sı (%92.8)Escherichia coli’ydi.Sonuç: Sonuç olarak, yaşlı nüfus artışı ve yaşla birlikte alttayatan hastalıklarda artış olmasının enfeksiyon hastalıklarıaçısından sorunlara neden olacaktır. Bu yaş grubundaenfeksiyonların klasik belirti ve bulgularının görülmeyeceğiakılda tutulmalıdır
Abstract
Objective: Infectious diseases may course with different symptoms in geriatric patients compared with younger adults and delays in the diagnosis and treatment may be experienced. In this study, it was aimed to evaluate the infectious diseases in elderly patients who were followed up in our clinic. Material and Methods: The data of patients aged ≥ 65 years who were hospitalized in the Infectious Diseases and Clinical Microbiology Clinic were evaluated retrospectively. Demographic characteristics, underlying diseases, length of their hospital stay, laboratory findings, culture results and diagnoses were recorded. Results: Ninety-six patients aged ≥ 65 years were included in the study. Mean age of the patients was 76.5±7 (range 65-98) years. The underlying diseases were hypertension (23.5%), diabetes mellitus (21%), chronic renal failure (11%), chronic obstructive respiratory disease (10%) and malignancy (9.2%). The most frequent cause of hospital admission was urinary tract infection (53.5%), followed by soft tissue infection (18.8%), pneumonia (8%) and acute gastroenteritis (5%). The median length of hospital stay was 8.5 (interquartile ranges [IQR]: 6-12.8) days. Febrile episodes were detected in 73% of the patients. Mean sedimentation rate was 62±33.3 mm/h. Median leukocyte count and C-reactive protein (CRP: 0.05). There was no significant difference between urinary system, soft tissue infections, pneumonia, acute gastroenteritis, and other infections as for fever, leukocytosis and CRP elevation (p>0.05). Thirty-one of the 38 (81.6%) culture samples obtained from the patients were urine specimens. The most commonly isolated bacteria were Escherichia coli (92.8%) in the urine specimens. Conclusion: In conclusion, the increase in the elderly population and the underlying diseases with age will cause problems in terms of infectious diseases. It should be kept in mind that the classical signs and symptoms of infections will not be seen in this age group
