Fetal Anomali ve Hidrops Fetalis Tanısı Almış Gebelerde Real Time PCR Yöntemi ile Human Parvovirüs B19 Pozitifliğinin Araştırılması

Investigation of Human Parvovirus B19 Positivity by Real-Time PCR Method in Pregnant Women with Fetal Anomaly and Hydrops Fetalis

Özet

Amaç: Bu çalışmada, fetal anomali ve hidrops fetalis tanısı almış gebelerde Parvovirüs-B19 DNA pozitifliğini retrospektif olarak araştırarak; fetal enfeksiyon, virüs ve viral yük ile ilişkisinin incelenmesi amaçlanmıştır. Yöntem: Çalışmaya Temmuz 2014-Mart 2018 tarihleri arasında laboratuvarımıza gönderilen on bir hastaya ait; on iki klinik örnek dâhil edildi. Parvovirüs B19 IgM/IgG antikorları ELISA yöntemi (NovaTec, Almanya) ile değerlendirildi ve Real-Time PCR yöntemi (LightMix® Kit Parvovirus B19, Roche, Almanya) ile Parvovirüs-B19 DNA varlığı kantitatif olarak araştırıldı. Bulgular: Klinik örneklerde %16.7 (2/12) Parvovirüs-B19 DNA pozitifliği belirlendi. Fetal anomali saptanmış 22 haftalık gebeden eşzamanlı alınan amniyon sıvısı ve serum örneklerinden amniyon sıvısında 106kopya/ml, serum örneğinde 10³ kopya/ml Parvovirüs-B19 DNA saptandı, serolojik analizinde Parvovirüs-B19 IgM negatif olarak belirlendi. Parvovirüs-B19 DNA negatif üç gebede IgG pozitifken IgM negatif bulundu; bir hastada IgG ve IgM negatif bulundu; bir hastada IgG negatifken, IgM “grey zone” olarak değerlendirildi, bir hastaya ait serolojik analiz sonuçlarına ulaşılamadı. IgG negatifken IgM 2-4 hafta sonra testin yinelenmesi önerilerek, grey zone olarak değerlendirilen hastada 8. haftada düşük gerçekleşti. Parvovirüs-B19 DNA saptanan hastanın kromozom analizi sonucunun normal olarak değerlendirildiği ve bebeğin sağlıklı bir şekilde dünyaya geldiği görüldü. Çalışmaya dâhil edilen diğer gebelerden üçünün gebeliğinin fetal anomali nedeniyle sonlandırıldığı, iki hastanın hidrops fetalis, iki hastanın fetal distres, bir hastanın fetal hidrotoraks tanısı aldığı, iki hastada ise gebeliğin normal olarak sonlandığı görüldü. Sonuç: Serolojik analiz sonuçları negatif olsa bile Real-Time-PCR yöntemi ile 10 kopya/ml’ye kadar ParvovirüsB19 DNA’sı saptanabilmekte ve kantitatif sonuç verilebilir. Gebelerde Parvovirüs-B19 DNA varlığının ve miktarının Real-Time PCR yöntemi ile saptanmasının, fetal Parvovirüs-B19 enfeksiyonunun erken tanısı ve takibi açısından önemli olacağı unutulmamalıdır.

Abstract

Objective: The aim of this study was to retrospectively investigate the presence of Parvovirüs B19-DNA in pregnant women diagnosed with fetal anomaly and hydrops fetalis and its association with fetal infection and viral load. Method: Twelve clinical samples of eleven patients referred to our laboratory between July 2014-March 2018 were included in the study. Parvovirüs B19-IgM and IgG antibodies and were evaluated by ELISA (NovaTec, Germany) and the presence of Parvovirus-B19 DNA was quantatively investigated by Real-Time PCR (LightMix® Kit Parvovirus B19, Roche, Germany). Results: Parvovirus-B19 DNA-positivity was identified in 2 of 12 (16.7%) samples. Parvovirus-B19 DNA was detected in both amniotic fluid and serum samples, which were simultaneously taken from 22-week pregnant women with fetal anomalies with 106 copies/ml and with 10³ copies/ml, respectively. Parvovirus-B19 was detected as IgM negative in serological analysis. When IgG was negative, serological test for IgM was recommended to be repeated 2-4 weeks later, and a miscarriage occurred in the 8th gestational week of a woman whose IgM was evaluated as being in “grey zone”. Chromosome analysis of one of the patients, whom ParvovirusB19 DNA was detected, was considered normal and a healthy baby was born. Pregnancy was terminated in three other pregnants included in the study due to the presence of fetal anomalies. The pregnancies were terminated due to the diagnosis of hydrops fetalis in 2, and fetal distress in 2, fetal hydrothorax in 1 patient, while two patients had term deliveries. Conclusion: Even if the results of serological analysis were negative, Parvovirus B19-DNA can be quantitatively detected up to 10 copies/ml by Real-Time-PCR. It should not be forgotten that detection of the presence and viral load of Parvovirus B19-DNA by Real-Time PCR is quite important in terms of early diagnosis of fetal Parvovirus B19 infection in pregnancies.
