Pilonidal Apseli Olgularda Unroofing-Curettage ve Sekonder İyileşme Yöntemine Nanova(TM) Therapy System Kullanımının Etkisi

Özet
Amaç: Pilonidal sinüs hastalığının asemptomatik, kronik fistülizan ve nüks formlarında bir tedavi şekli oturmuş olmakla birlikte apseli başvuran hastalarda nihai tedavinin ne zaman yapılması gerektiği konusunda fikir birliği yoktur. Bu çalışmamızda unroofing-curettage yapılan pilonidal apseli hastalarda iyileşmeyi hızlandırmak için kullanılan "elektrik enerjisi gerektirmeyen negatif basınç yardımlı yara pansumanını" sunmayı amaçladık.Yöntem: Kliniğimizde Haziran-Aralık 2015 tarihleri arasında pilonidal apse ile başvuran ve "unroofing-curettage ve sekonder iyileşme" ile birlikte "elektrik enerjisi gerektirmeyen negatif basınç yardımlı yara pansumanı" yapılan beş hastanın verileri çalışmaya alındı. Hastaların yaşı, cinsiyeti, tedavi süreleri, komplikasyonlar ve nüks oranları incelendi. Apse ile başvuran hastalara lokal anestezi altında drenaj işlemi yapıldıktan sonra unroofingcurettage yapıldı. Açık kalan alana ilk 3 uygulama 48 saatte bir, sonraki uygulamalar ise 72 saatte bir olmak üzere "Nanova(TM) Therapy System" kullanılarak pansuman yapıldı.Bulgular: Olgu sayısı az olduğu için hastalar tek tek incelendi. Hastaların yaş ortalaması 33 yıl (28-38 yıl) olup tüm hastalar erkekti. Hastaların anamnezinde daha önce pilonidal sinüs öyküsü yoktu. Bir hastaya 3, üç hastaya 4 ve bir hastaya da 5 kez uygulandı. Ortalama uygulama sayısı 4 olarak saptandı. Sadece bir hastada komplikasyon olarak kanama gelişti. Ortalama iyileşme süresi 22,2±6,26 gündü. Ortalama takip süresi 20,8±3,34 ay olup bu süre zarfında nüks saptanmadı. Sonuç: Çalışmamızda elektrik enerjisine ihtiyaç duymayan el pompası yardımı ile negatif basınç uygulayan yara pansumanı kullanılmıştır. Hasta sayısı az olduğundan istatistiki bir karşılaştırma yapılamamıştır. Ancak daha önceki çalışmalara kıyasla sekonder iyileşmeye bırakılan hastalarda vakum destekli pansumanın iyileşme süresini kısalttığını tespit ettik.
Abstract
Aim: Although treatment methods have been well-established for asymptomatic, chronic fistulizing, and recurrent forms of pilonidal sinus disease, there is no consensus on when definitive treatment should be performed in patients with abscesses. In this study, we aimed to present non-powered negative-pressure wound dressing used to facilitate healing in patients with pilonidal abscesses who underwent unroofing-curettage.Method: The data of five patients who presented to our clinics between June-December 2015 with pilonidal abscesses and underwent unroofingcurettage and secondary healing with non-powered negative-pressure wound dressing were included in the study. The patients' age, sex, duration of treatment, complications, and recurrence rates were analyzed. Unroofing-curettage was performed after drainage under local anesthesia. The wound was dressed using the "Nanova(TM) Therapy System", applying the first 3 treatments every 48 hours, followed by the next applications once every 72 hours.Results: Since the number of cases was small, the patients were examined one by one. The mean age of the patients was 33 years (28-38 years) and all patients were male. There was no previous history of pilonidal sinus in the patients' medical histories. Wound dressing was applied 3 times to one patient, 4 times to three patients, and 5 times to one patient (average, 4). Only one patient developed hemorrhage as a complication. Mean recovery time was 22.2±6.26 days. The mean follow-up period was 20.8±3.34 months and no recurrence was detected during this period. Conclusion: In our study, we used negative-pressure wound dressing by means of a non-powered hand pump. Since the number of patients was small, a statistical comparison was not possible. However, we found that vacuum-assisted dressing shortens the healing period compared to previous studies in patients whose wounds were left to heal by secondary intention.
