Özet

İkiboyutlu renkli Doppler ekokardiyografi, ventriküler septal defektlerin (VSD) tanı ve izleminde temel görüntüleme yöntemidir. Ventriküler septal defektlerde prognozu belirleyen önemli faktörlerden defektin yeri, çapı, soldan sağa şant miktarı ve pulmoner arter basıncı hakkında ekokardiyografiyle güvenilir bilgi elde edilebilmesi, preoperatif değerlendirmede rutin kalp kateterizasyonu gereksinimini ortadan kaldırmıştır. Günümüzde üçboyutlu ekokardiyografinin kullanıma girmesiyle VSD’nin şekil ve boyutları, komşu yapılarla uzaysal ilişkileri daha da iyi değerlendirilebilmektedir. Ventriküler septal defektlerde ekokardiyografik inceleme segmental yaklaşımla yapılmalı, intra- ve ekstrakardiyak yapılar sistematik olarak değerlendirilmeli, ventriküler septum çeşitli kesitlerden incelenmelidir. Bu yazıda, Avrupa Pediyatrik Kardiyak Kodu’nda benimsenen sınıflandırma ve terminoloji doğrultusunda VSD’lerin farklı tiplerinin morfolojik özellikleri ve bunların ekokardiyografik olarak nasıl ayırt edileceği üzerinde durulmuş; ekokardiyografi ile defektin yerleşimi, çapı, hemodinamik özelliklerinin değerlendirilmesi, seyir ve izleminde dikkat edilmesi gereken noktalar ele alınmıştır.

Abstract

Two-dimensional color Doppler echocardiography is the primary imaging modality in the diagnosis and follow-up of ventricular septal defects (VSD). It provides detailed and reliable information on important prognostic determinants such as defect morphology, localization, size, amount of left-to-right shunt, and pulmonary artery pressure, abolishing the need for routine preoperative diagnostic cardiac catheterization in this group of patients. With the advent of three-dimensional echocardiography, the size, position, and spatial relations of VSDs with other cardiac structures can now be more accurately depicted from en face views. The echocardiographic examination of VSDs should include systematic evaluation of intra- and extracardiac structures utilizing the segmental approach, and because of its complex, nonplanar configuration, the ventricular septum should be visualized from multiple planes. This review focuses on the echocardiographic features that distinguish different morphological types of VSDs in accordance with the classification and terminology accepted by the European Paediatric Cardiac Code, assessment of the size and hemodynamic properties of such defects, and addresses certain issues encountered during their course and follow up.
