Tam kapalı blalock-taussig şanta stent uygulaması: olgu sunumu

Özet

Özet – Sistemik-pulmoner şantlara veya duktus arteriyozusa stent takılması nispeten yeni ve sık yapılmayan bir girişimdir. Fallot tetralojisi tanısıyla modifiye Blalock- Taussig şantı yapılmış olan 22 aylık bir erkek çocukta, ciddi siyanoz nedeniyle yapılan ekokardiyografik incelemede şantın tam kapalı olduğu görüldü. Balon anjiyoplasti ile açılan şantın akut tromboz ile hemen yeniden tıkanması üzerine, stent yerleştirilerek şantın tam açılması sağlandı. Başlangıçta %32 olan oksijen doygunluğu işlem sonrasında %92’ye yükseldi. Hasta aspirin tedavisiyle taburcu edildi.

Abstract

Summary – Stent implantation into a ductus arteriosus or systemic-pulmonary shunt is a relatively new but infrequent approach for palliation. A 22-month-old boy with a modified Blalock-Taussig shunt for tetralogy of Fallot was admitted with severe cyanosis. Echocardiographic examination showed complete occlusion of the shunt. Initial balloon angioplasty resulted in reocclusion of the shunt by acute thrombosis. Finally, recanalization was achieved by stent implantation. Oxygen saturation of the patient increased from 32% to 92% following stenting. He was discharged on aspirin therapy.
