Percutaneous closure of perimembranous ventricular septal defects associated with septal aneurysm in adults

Özet
Amaç: Perimembranöz ventriküler septal defektlerin (pmVSD)perkütan olarak kapatılması seçilmiş olgularda cerrahi kapat- maya seçenek olarak kabul görmüştür. Ancak septal anevriz- manın eşlik ettiği pmVSDlerin kapatılması daha zorlayıcıdır.Bu makalede septal anevrizmanın eşlik ettiği pmVSDlerin ci- haz ile kapatılması ile ilgili deneyimlerimizi bildirdik. Yöntemler: Merkezimizde 2008-2012 yılları arasında septalanevrizmanın eşlik ettiği pmVSDsi olan 11 olguda perkütankapatma girişiminde bulunuldu. İşlem sonrası 1., 3., 6. ve 12.aylarda hastaların klinik takipleri yapıldı. Bulgular: Hastaların ortalama yaşı 36.2±1.3 (%64 erkek,%36 kadın) idi. Ventrikülografide interventriküler septumdakianevrizmanın sol ve sağ ventriküle olan açıklıklarının çap- ları sırası ile 13.5±5.6 mm ve 5.9±2.2 mm idi. Defektin ka- patılmasında 4 hastada Amplatzer pmVSD oklüder cihazı, 4hastada Amplatzer musküler ventriküler septal defekt oklüdercihazı, 1 hastada Amplatzer Duct oklüder-I cihazı, 2 hastadaise Amplatzer Duct oklüder-II cihazı kullanılması tercih edildi.Bütün hastalarda işlem başarılı olarak tamamlandı. Hasta- lar işlem sonrası ortalama 22±1.9 ay takip edildi. Erken veyaorta dönem takipte cihaz veya işlemle ilişkili hiçbir komplikas- yon gelişmedi. Sonuç: Anevrizmanın eşlik ettiği pmVSDlerde perkütan ka- patma basit defektlere göre daha zorlayıcıdır. Cihaz tipinin veboyutunun seçimi anevrizma ve defektin şekline ve boyutunagöre yapılmalıdır.
Abstract
Objective: Percutaneous closure of perimembranous ven- tricular septal defects (pmVSD) has become an acceptedalternative to surgical closure in selected cases. However,closure of pmVSDs associated with septal aneurysm is morechallenging. We report our experience of device closure ofpmVSDs associated with septal aneurysm. Methods: Between 2008 and 2012, percutaneous closure ofpmVSD associated with septal aneurysm was attempted in 11adult patients in our institution. The patients were followed upat 1, 3, 6, and 12 months after the procedure. Results: Mean age of the patients (64% male, 36% female)was 36.2±1.3 years. Diameter of the left and right ventricularopenings of the aneurysm measured by ventriculography was13.5±5.6 mm and 5.9±2.2 mm, respectively. The defect wasoccluded with Amplatzer pmVSD Occluder in 4 patients, Am- platzer Muscular Ventricular Septal Defect Occluder in 4 pa- tients, Amplatzer Duct Occluder I in 1 patient, and AmplatzerDuct Occluder II in 2 patients (AGA Medical Corp., Plymouth,MN, USA). The procedure was succesfull in all patients. Meanfollow-up time was 22±1.9 months. There was no device- orprocedure-related complications at the acute setting or mid- term follow-up. Conclusion: Percutaneous closure of pmVSDs associatedwith aneurysm is more challenging than that of simple de- fects. The selection of the device type and size should bemade according to the configuration and size of the aneurysmand defect.
