Özet

Myom ve fertilite etkileşimi üzerine tartışmalar uzun bir süredir devam etmektedir. Son derece tartışmalı bir konu üzerine halen fikir birliğine varılamamasının esas nedeni ise randomize, kontrollü çalışmaların eksikliğinden kaynaklanmaktadır. ‹lk bakışta mevcut retrospektif-gözlemsel ve az sayıdaki prospektif-gözlemsel çalışmalar irdelendiğinde myomun fertilite üzerine hem gebe kalabilme, hem de gebeliğin devamı açısından olumsuz bir etkisinin olabileceği söylenebilir. Submüköz veya endometrial kaviteye bası yapan intramural yerleşimli myomların varlığında bu etki daha belirgin iken endometriuma bası yapmayan intramural myom vakalarında bu etki mevcut olmakla birlikte daha zayıf bir bilimsel veriye dayanmaktadır. Eldeki literatür verileri incelendiğinde submüköz ve kaviteye bası yapan myom varlığında spontan veya yardımcı üreme teknikleri (YÜT) öncesi myomektomi nispeten rahatlıkla önerilirken, sadece intramural komponenti olan vakalarda rutin myomektomiyi önermek için yeterli bilgi birikimi mevcut değildir. Bu derlemede myomun spontan ve YÜT gebelik oranları ile ilişkisi ve gebeliğin devamı üzerine etkisi irdelenecek, tedaviye karar verilmesi durumunda hangi yaklaşımın hasta yararına olabileceği güncel çalışmalar ışığında tartışılacaktır.

Abstract

There is an ongoing debate about the effect of leiomyomata on reproductive outcome. The absence of randomized controlled trials is the main reason why this hot topic has been still actual. Generally, based on the available literature, it may be suggested that leiomyomata may have an adverse effect on both fertility and the surveillance of the pregnancy. Although this deleterious effect is more obvious in the presence of submucous or intramural leiomyomata with distortion of the endometrial cavity, this association is based on more inconclusive data in cases with intramural leiomyomata without endometrial distortion. Therefore, while myomectomy should be recommended more precisely in patients with submucous or intramural leiomyomata with distortion of the endometrial cavity before spontaneous conception or assisted reproduction technologies (ART), there is paucity of data to recommend routine treatment in the remaining cohort of patients. In the current review, the impact of leiomyomata on the reproductive performance has been discussed in terms of patients undergoing ART or attempting spontaneous conception.
