Hiperkalseminin pankreas morfolojisi ve pankreatik kanal geçirgenliği üzerine etkisi: Deneysel çalışma

Özet
Akut pankreatit etyolojisinde rol oynayan hiperkalseminin pankreatite yol açma mekanizması üzerine çeşitli teoriler vardır. Pankreatik kanal membran geçirgenliğinin bozulması akut pankreatit patogenezinde suçlanan ana teorilerden biridir. Bu çalışma 6'şar kediden oluşan 3 grup üzerinde yapıldı. Kontrol grubu olan grup I'de splenik arterden serum fizyolojik infüzyonu yapılırken, baş ve kuyruk kesiminde kateterize edilen anapankre-atik kanalda yapılan perfüzyonda kanalın klor iyonlarına karşı geçirgenliğine bakıldı. Grup 11 ve IlI'de serum fizyolojik yerine kalsiyum glukonat infüzyonu yapıldı, grup lirde ek olarak belirli aralıklarla bir kalsiyum ka-nal bloken olan verapamil verildi. Toplam 18 kedi üzerinde bu yaptığımız çalışmada, hiperkalseminin pank-reas morfolojisi üzerine olan etkisini, bu sırada pankreatik kanalın klor iyonuna olan geçirgenliğini ve kalsi-yum kanal blokerinin bu parametreleri etkileyip etkilemediğini araştırdık. Çalışmada akut hiperkalsemi mor-folojik ve histopatolojik olarak akut pankreatit oluşturduğu gözlendi. Abstract
There are various theories dealing with the mechanism by which hypercalcemia leads to acute pancreatitis. Dis-ruption of the pancreatic channel membrane permeability is one of the four theories about acute pancreatitis pathogenesis. The effect of acute hypercalcemia on the morphology and ductal permeability of cat pancreas was investigated. The study was performed on three groups each containing 6 cats. In group 1, as the control gro-up, serum physiologic (SP) infusion was done via splenic artery and pancreatic ductal perfusion study was car-ried out by catheterizing the main pancreatic channel. In group 2 and 3, calcium gluconate was infused ins-tead of SP and in group 3, an additional periodic administration of calcium channel blockers was done. In this study, performed on 18 cats, we investigated the effects of hypercalcemia on pancreas morphology, the chloride ion permeability of the pancreatic channel and whether calcium channel blockage could influence these parameters. In this experimental study, acute hypercalcemia was shown to cause acute edematous, and mode-rate necrotizing pancreatitis both macroscopically and microscopically. No significant effect of calcium chan-nel blockage were detected at the given doses and time intervals. Net chloride ion flux in the pancreatic chan-nel was compared between control and study groups and a statistically significant difference was detected, emphasizing the disruption of the pancreatic channel permeability. These results suggest that hypercalcemia ta-kes a part in the ethiopath agenesis of pancreatitis by changing pancreatic channel permeability.
