Özet
Akciğer adenoskuamoz karsinomu (ASK) akciğer kanserlerinin nadir rastlanan bir formudur ve klinik patolojik özellikleri iyi tanımlanmamıştır. Insidansının %0.4-4 arasında değiştiği bildirilmektedir. Bu çalışmada 1990-1996 yılları arasında ASK tanısı alan 25 hastanın klinik, patolojik ve biyolojik davranış özelliklerinin saf adenokarsinom ve epidermoid karsinom ile karşılaştırması amaçlandı. Primer akciğer kanserli hastalar içinde ASK'lı hastaların görülme sıklığı %1.9 olarak bulundu. Sigara öyküsü 22 hastada (%88) mevcuttu. Hastaların 22'si erkek, 3 'ü kadın ve, medyan yaşı 56 (42-75) idi. ASK'lı hastalarda erkek-kadın oranı ve kitlelerin lokalizasyonları saf adenokarsinoma benziyordu. Bu çalışmada ileri evre (evre IIIB ve IV) hasta sayısı ASK grubunda saf adenokarsinom ve epidermoid karsinom grubuna göre daha yüksek (sırası ile %84, %72 ve %79) bulundu. Ayrıca ASK'lı grupta komplet rezeksiyon uygulanan hastalar (%16), diğer hastalara göre (adenokarsinomda %20 ve epidermoid karsinomda %26) daha düşük oranda bulundu. Komplet rezeksiyon uygulanan 4 ASK'lı hastada 5 yıllık sağkalım sağlandı. Abstract
Adenosquamous (ASC) carcinoma of the lung is an uncommon form of the lung cancer, the clinicopathologic features of which are not well defined. The incidence of adenosquamous carcinoma varies from 0.4% to 4% in previously published reports. In this report, we clinicopathologically analyzed 25 ASC cases between 1990-1996 to clarify its biological behavior in comparison with that of adenocarcinoma and squamous cell carcinoma of the lung. The frequency rate of ASC were 1.9%. In ASC group, there were 22 male and 3 female patients and median age was 56 (range, 42-75). A smoking history was available in 22 patients (88%). Our results showed that the sex ratio and location of ASC were similar to adenocarcinoma. In the present study, the percentage of patients with ASC who showed an advanced pathologic stage (IIIB and IV) (84%) was higher than that of patients with adenocarcinoma (72%) and squamous cell carcinoma (79%). In addition, the rate of complete resection was lower in patients with ASC (16%) than in patients with squamous cell carcinoma (20%) and adenocarcinoma (26%). Five-year survival was obtained in four patients with complete resections. Median survival of patients with ASC were statistically poorer than patients with squamous cell carcinoma and adenocarcinoma (p< 0.05). Although our series is small, this study suggests that ASC of the lung is an aggressive tumor with a rapid growth.
