Bebeklerde oral kandida kolonizasyonu üzerine etkili faktörler

Özet

Bu çalışmada anne memesi veya biberon ile beslenen bebeklerde oral kandida kolonizasyon sıklığının farklı olup olmadığı bir yaş altında olan 184 bebek üzerinde araştırılmıştır. Bebeklerin tümünün oral sürüntü örnekleri, anne memesi ile besleniyorsa annenin meme başı sürüntü örneği, biberon veya yalancı emzik kullananlarda plastik yüzeyden sürüntü örnekleri alınarak mikolojik yöntemlerle incelenmiştir. Bulgularımıza göre kız bebeklerde oral kandida kolonizasyonu %26.0, erkek bebeklerde %27.9 oranında bulunmuştur (p>0.05). Yaş gruplarına göre incelendiğinde, ilk 3 aylık bebeklerde oral kandida kolonizasyonu (%39) sonraki aylara göre daha yüksek bulunmuştur (p=0.008). Bebekler beslenme şekillerine göre incelendiğinde ise sadece anne memesi ile beslenen bebeklerde oral kandida kolonizasyon oranı %26.5, sadece biberon ile beslenenlerde %29.6 olarak bulunmuştur. Karışık beslenenlerde bu oran %27.2'dir (p>0.05). Bebekler emzik alışkanlıklarına ve uygun beslenme temizliği yapılıp yapılmadığına göre de incelenmiştir. Oral kandida kolonizasyonu emzik alışkanlığı olanlarda %35.1, olmayanlarda %15.0 (p=0.002); beslenme temizliği yapılmayanlarda %41.8, yapılanlarda ise %22.4 (p=0.029) oranlarında bulunmuştur. Ailenin sosyoekonomik düzeyi ile bebeklerde oral kandida kolonizasyonu arasında ilişki bulunmamıştır. Üretilen maya türlerinin %74' ü Candida albicans' tır. Sonuç olarak bebeklerde oral kandidozun önlenmesinde annelerin beslenme temizliği, özellikle meme başı temizliği konusunda eğitilmesinin ve emzik alışkanlığının azaltılmasının önemli olduğu düşünülmüştür.

Abstract

The aim of this study was to determine whether there were any differences in oral candidal colonization between breast fed and bottle fed infants. The study was performed on 184 infants under the age of one. Oral swap samples of all infants, mothers' nipple swaps if they were breast-fed and plastic surface swaps if they used bottles or pacifiers were taken and investigated by mycological methods. According to the results, the rate of oral candidal colonization was 26.0% in female infants and 27.9% in male infants (p>0.05); and it was higher in infants in their first 3 months (39.0%, p=0.008). As to the way of feeding, the rate of oral candidal colonization was 26.5% in breast-fed infants, 29.6% in bottle-fed infants and 27.2% in infants fed by both of them (p>0.05). It was also investigated whether infants used pacifiers and whether sufficient care was given to the hygiene of nutrition. Oral candidal colonization was 35.1% and 15.0% in infants using pacifiers and not using them, respectively (p=0.002). The rates of oral candidal colonization in infants with sufficient and insufficient nutritive hygiene were 41.8% and 22.4%, respectively. No relation was found between the socioeconomic status of the family and the oral candidal colonization of infants. Seventy four per cent of the yeast species isolated was Candida albicans. As a result, education of mothers about hygienic care in nutrition, particularly care of breast nipple and reduction of pacifier usage are the important factors in the prevention of oral candidal colonization.
