Özet

Amaç: Tip 2 diabetes mellitus (Tip 2 DM) hastalarında mikro ve makroanjiyopatilerin gelişmesi genetik yapıdan etkilenmektedir. Bu anjiyopatiler içerisinde en büyük oranda koroner, renal ve retinal arterlerin etkilenmesi yer almaktadır. Anjiyotensin I konverting enzimi (ACE), anjiyotensin U oluşumunu ve kinin katabolizmasını sağlayarak sistemik ve renal kan dolaşımı üzerinde etkili olmaktadır. Plazma ve hücresel ACE düzeyleri genetik olarak ACE geni insersiyon (I)/delesyon (D) polimorfizmi ile belirlenmektedir. I allelinin homozigotu (genotip II) olan bireylerde plazma ACE seviyesi en düşük olarak saptanmaktadır, Yöntem: Biz bu çalışmamızda Tip 2 DM olgularında gelişen nefropati komplikasyonunun ACE/ID polimorfizmi ile olan ilişkisini inceledik. Çalışmaya makro veya mikrovasküler, komplikasyon tespit edilmeyen 46 Tip 2 DM'li hasta, Tip 2 DM'li ve makroproteinüri tespit edilen 37 olgu alındı ve 145 sağlıklı olgu kontrol grubunu oluşturdu. Bulgular: Komplikasyonsuz seyreden Tip 2 DM olgularında ACE D allelini kontrol grubuna göre anlamlı derecede yüksek tespit ettik. Nefropati gelişen Tip 2 DM grubunda ise I veya D alleline ait dominant bir etki saptamadık. Yorum: Türk popülasyonunda yapılan bu çalışmanın sonuçlarına göre komplikasyonsuz seyreden Tip 2 DM olgularında ACE D allelinin etkisi bulunduğu fakat bu etkinin nefropati komplikasyonu gelişimine ayrıca bir katkı sağlamadığı bulunmuştur.

Abstract

Objective: Development of micro and macroangiopathic complications is effected by genetic structure in patients with Type 2 diabetes mellitus (Type 2 DM). Coronary, renal and retinal vasculature are the most frequently involved among the angiopathies. Angiotensin I convening enzyme (ACE) regulates systemic and renal circulation by facilitating angiotensin II production and catabolism of kinins. Plasma and intracellular ACE levels are determined genetically by the insertion (I)/deletion (D) polymorphism. Subjects homozygote for I allel (genotype II) are those with the lowest ACE levels. Method: In this study we looked for the relation between the ACE/ID polymorphism and development of nephropathy (NP) complication in patients with Type 2 DM. Type 2 DM patients without any complications (1st group, n= 46), Type 2 DM patients with nephropathy (2nd group, n= 37), and 145 healthy control subjects without any chronic disease including DM and coronary artery disease are involved in the study. Results: We observed a higher frequency of D allel in Type 2 DM patients without any complications than the healthy controls. We could not observe a dominant effect of either I or D allel in Type 2 DM patients with NP. Conclusion: The results of our study performed on Turkish population suggest that ACE D allel has an effect on Type 2 DM, but does not influence development of nephropathy in Type 2 DM.
