Seçilmiş bir hipertansif hasta grubunda yüksek sekonder hipertansiyon sıklığı ve tanısal yaklaşım

Özet

Amaç: Hipertansif hastaların yaklaşık %10'unda başta renal ve endokrin nedenler olmak üzere çeşitli sekonder etyolojiler sözkonusudur. Çalışmamızda seçilmiş bir hipertansif hasta grubunda sekunder neden sıklığının ve biyolojisinin araşlınlması ve bu arada sekunder hipertansiyona tanısal yaklaşımın gözden geçirilmesi amaçlandı. Yöntem: Çalışmaya, yaşları 18-50 arasında değişen, takip eden kriterlerden an az birini taşıyan 50 hipertansif olgu alındı; 30'un altında yaş, yeni hipertansiyon olup diastolik kan basıncı $geq$ 110 mmHg, en az iki antihipertansif ilaçla yapılan tedaviye dirençli hipertansiyon. Bulgular: Oniki (%24) hastada sekonder hipertansiyon nedeni saptandı (renalparankimal hastalık 5 (%10), primer aldosteronizm 2 (%4), renovasküler 1 (%2), akut glomerülonefrit l (%2), feokromositoma 1 (%2), Cushing hastalığı 1 (%2), aort koarktasyonu 1 (%2). Yorum: Çalışmamızdaki seçilmiş grupta sekonder hipertansiyon sıklığı %24 gibi yüksek bulundu. Renal parankimal hastalıklar sekonder hipertansiyonun halen en sık nedenidir. Sekonder hipertansiyon olgularında spesifik tedavi mümkün olabileceğinden seçilmiş hipertansif olgularda sekonder nedenler mutlaka araştırılmalıdır.

Abstract

Objective: Approximately in 10% of hypertensive patients secondary causes (mainly renal and endocrin) are detectable. The aim of the study was to examine the frequency and etiology of secondary causes in a selected hyertensive patient group and to evaluate the diagnostic approach to secondary hypertension. Method: Fifty hypertensive patients, aged between 18-50, with at least one of the following criteria were included into the study; age below 30, recent diagnosed hypertension with a diastolic blood pressure more than 110 mmHg, hypertension refractory to antihypertensive therapy with at least two drugs. Results: Secondary hypertension was diagnosed in 12 (24%)patients renal parenchymal disease 5 (10%), primary aldosteronism 2 (4%), renal vascular 1 (2%), acute glomerulonephritis 1 (2%), pheocromocytoma 1 (2%), Gushing disease 1 (2%), coarctation of the aorta 1 (2%). Conclusion: We found a high frequency (24%) of secondary hypertension in this selected group. Renal parenchymal disease is still the most common cause of secondary hypertension. Because the specific treatment of secondary hypertension may be possible, in selected cases the secondary causes of hypertension should be carefully investigated.
