Behçet hastalığında üst Gastrointestinal sistem tutulumu ve bu hastalarda Helicobacter pylori sıklığı

Özet

Amaç: Behçet hastalığı patogenezinde; çeşitli bakterilerin antijenik özelliklerinin, insan doku proteinleriyle gösterdiği moleküler benzerlikler nedeniyle rolü olduğu düşünülmektedir. Bu çalışmanın amacı; Behçet hastalığı tanısı konulan hastalarda üst gastrointestinal sistem lezyonlan ve bu hastalarda Helicobacter pylori sıklığını belirlemektir. Yöntem: Uluslararası Çalışma Grubu kriterlerine göre, Behçet hastalığı tanısı alan 62 olgu çalışma grubu, sağlıklı 40 olgu kontrol grubu olarak alındı. Çalışma ve kontrol grubu olgulann özefagogastroduodenoskopisi yapıldı, işlem sırasında antrum, korpus ve lezyon etrafından çoklu biyopsiler alınarak histopatolojik değerlendirme ve H. pylori'nin varlığı araştınldı. Bulgular: Çalışma grubundaki 62 hastanın endoskopik incelemeleri sonucu 4 (%6.6)'ünde özefajit, 2 (%3-3)'sinde özefagus varisi, 28 (%45.2)'inde kronik antral gastrit, 4 (%6.6)'ünde gastrik aftöz ülser, 2 (%3-3)'sinde duodenal aftöz ülser tespit edildi. Kontrol grubunda ise 40 hastanın 2 (%5)'sinde özefajit, 13 (%32.5)'ünde kronik antral gastrit, 2 (%5)'sinde gastrik ülser ve l (%2.5)'inde duodenal ülser tespit edildi. Çalışma grubunda oral aftöz ülseri olan 43 olgunun 39 (%90.6)'unda ve oral aftöz ülseri olmayan 19 olgunun 9 (%47.3)'unda H. pylori infeksiyonu olduğu gözlendi (p< 0.001). Yorum: Çalışmamızda Behçet hastalannda üst gastrointestinal sistemde oral aftöz lezyonlan görülen hasta-lardaki H. pylori infeksiyonu, oral aftöz lezyonlan tespit edilmeyen hastalara göre daha sık bulunmuş olup, bu sonuç H. pylori infeksiyonunun Behçet hastalannda oral aftöz ülserlerin oluşumuyla ilişkili olabileceğini düşündürmektedir.

Abstract

Objective: It is thought that molecular similarities of human tissue antigens with some bacterial antigens are important in path agenesis of Behçet's disease. Our aim in this study was to find the frequency of upper gastrointestinal system lesions and Helicobacter pylori infections in Behçet's patients.Method: The study group consisted of 62 patients and the control group consisted of 40 healthy people. The diagnosis ofBehçet's disease in these patients has been done according to the criteria of international study group. In all 62 Behçet's patients and 40 healthy people, esophagogastroduodenoscopy was performed. During the esophagogastroduedonoscopy, multiple biopsies were taken from antrum, corpus and lesions surrounding for histopathologic evaluation and to find presence of H. pylori infection. Results: By means of endoscopie evaluation in 4 (6.6%) patients esophagitis, in 2 (3-3%) patients esophagial varices, in 28 (45.2%) patients chronic antrat gastritis, in 4 (6.6%) patients gastric aphthous ulcers, and 2 (3-3%) patients duodenal aphthous ulcers were found among 62 patients of the study group. In 2 (5%) patients esophagitis, in 13 (32.5%) patients chronic antrat gastritis, in 2 (5%) patients gastric ulcer and in 1 (2.5%) patient duodenal ulcer were found among 40 patients of the control group. H. pylori frequency was 77,4% in the study group, whereas 80.6% in the control group (p> 0.05). In the study group, H. pylori was positive in 39 (90.6%) cases among oral aphthous ulcer positive 43 cases, whereas it was positive in 9 (47.3%) cases among oral aphthous ulcer negative 19 cases (p< 0.001). Conclusion: In our study, we found that H. pylori infection was more frequently seen among oral aphthous lesion positive cases than oral aphthous lesion negative cases in Behçet's patients and this suggests that H. pylori may cause to occur oral aphthous lesion in Behçet's disease.
