RET proto-onkogen mutasyonu ve MEN 2A ailesi

Özet
MEN tip 2A sendromunda medüller tiroid karsinomu, feokromositoma ve primer hiperparatiroidizm ile birlikte görülür. RET proto-onkogen mutasyonlan, MEN 2A sendromu ve familial medüller tiroid karsinomuna neden olur. DNA analizi ile MEN 2A sendromu tanısı konularak total tiroidektomi yapılmasına, feokromositoma ve primer hiperparatiroidizm için takip ve tedaviye olanak tanır. Ataklar halinde hipertansiyon ve çarpıntı yakınmaları ile servisimize yatırılan 52 yaşındaki kadın hastanın fizik muayenesinde TA: 170/100 mmHg, nabız: 112/dakika kalp ritmik olarak bulundu. Feokromositoma yönünden araştırmaya alınan hastanın üst batın ultrasonografisinde sağ sürrenal lokalizasyonda 67 x 25 mm, sol sürrenalde 64 x 60 mm boyutlannda solid tümoral kitle tespit edildi. Hipertansif ataktan sonra toplanan 24 saatlik idrarda VMA 78 mg (3-9), metanefrin 6447 $mu$g (52-341), normetanefrin 5421 $mu$g (88-444) bulundu. I131 MIBG tüm vücut taramasında solda daha belirgin olmak üzere bilateral sürrenal lokalizasyonda tutulumu tespit edildi. Ötiroid nodüler guatr olan hastanın kalsitonin düzeyi 2000 pg/mL (0-50) olup tiroid ince iğne aspirasyon biyopsisi (TİAB) sonucu tiroid medüller karsinoma olarak rapor edildi. İntakt PTH düzeyi normal sınırlarda idi. MEN 2A tanısı alan hastaya bilateral sürrenalektomi yapıldı. Total tiroidektomi programına alınan hastanın kendisi, 6 çocuğu ve 4 torunu RET proto-onkogen açısından incelendi. Hastanın ve 1 oğlunun RET proto-onkogen analizinde 11. eksonun 634. kodonunda TGC$rightarrow$GGC heterozigot mutasyonu saptandı. Mutasyon saptanan oğlunda TİAB ile tiroid medüller karsinoma tespit edilerek total tiroidektomi uygulandı. Nadir tespit edilen MEN 2A sendromunda gen analizinin önemini vurgulamak için bu aileyi sunmayı uygun bulduk.
Abstract
In MEN type 2A syndrome, medullary thyroid carcinomas occurs in association with pheochromocytoma and primary hyperparathyroidism. RET proto-oncogen mutations are the cause of MEN 2 syndromes and familial medullary thyroid carcinomas. DNA analysis permits identification of MEN 2 syndromes and enables preventive total thyroidectomy and follow-up and treatment when needed for pheochromocytomas and primary hyperparathyroidism. 52 years old female patient with complaint of palpitation and episodic hypertensive attack was referred to our clinics. On physical examination blood pressure was 170/100 mmHg, pulse rate was: 112/min. The thyroid gland was grade 2 palpable with an 1 x 1.5 cm nodule in the right lobe. The patient went under investigation for pheochromocytoma and abdominal ultrasound revealed solid masses of 67 x 25 mm at right and 64 x 60 mm at left adrenal glands. Computed tomography of the abdomen revealed solid tumors of 6 x 5.5 cm at right and 7x6 cm at left adrenal gland. 24-hour urine collection after hypertensive attack was analyzed and results were as following; VMA: 78 mg(3-9) metanephrine: 6447$mu$g (52-341), normetanephrine: 5421 $mu$g (88-444). Whole body scan with I131 MIBG revealed pathological uptake at the locations of adrenal glands. Calcitonin level was 2000 pg/mL (N: 0-50) and the cytological examination of fine needle aspiration of thyroid nodule was reported as medullary thyroid carcinoma. PTH levels were normal. After diagnosis of MEN 2A, she underwent bilateral surrenalectomy and bilateral total thyroidectomy respectively. The analysis of the patient, her 6 children and her 4 grandchildren for mutations of the RET proto-oncogene by means of genomic poly-merase chain reaction revealed the heterozygote mutation of TCG$rightarrow$GGC in codon 634 of exon 11 in the patient and one of her sons. The cytological examination of fine needle aspiration of thyroid of her son was also reported as medullary thyroid carcinoma and he underwent bilateral total thyroidectomy. We present this family in order to emphasize the importance of screening of family genetic analysis in the rare syndrome MEN 2A.
