Özet

Quantitative Buffy Coat (QBC) yöntemiyle sıtma tanısı konvasiyonel yönteme (kalın damla) oranla daha büyük hacimde kanın görüntülenmesini sağladığından daha hassas bir yoldur. QBC sıtma saptama tüpleri, akridin oranj boyası ile potasyum oksalat , sodyum heparin ve dipotasyum EDTA gibi anti-koagulanların herhangi birisi ile sıvanmıştır (5, 6, 8). Tüpün içinde rahatça ayırt edilebilen , ayrılmış, ve genişlemiş katmanlar; açık ve koyu kırmızı hücreler (eritrositler), granülositler, lenfositler, monositler ve trombositlerdir (5, 6, 8). igne ucu kadar ufak olan ve flöresan veren infekte pozitif eritrositler infekte olmamış, eritrositler arasından ayırt edilirler (24, 31, 46). P. falciparum gametositleri orak biçimli sarı yapilar olarak bilinir. P. vivax'ın şizontları koyu kahverengi sıtma pigmentlerinin varlığı ile tanınırlar. Olgun şizontlarda büyüyen merozoitler koyu kahverengi pigmentlerle çevrelenmiş biçimde net olarak görülürler (5, 6, 8) Çalışmamızda orijinal uzunluğu 7.5 cm. olan kapilllar tüp 10 cm. uzatıldı ve 2 mm çap 0.3 mm, ye indirildi. Böylece uzunluk 2,5 cm. arttı, çap ise 6 kez kuçültüldü. Parazitemisi %0.03-1.7 arasında olan 237 P. vivax'lı hastaya ait kanlar incelendi. Degerlendirmeler (+), (++), (+ + +) ve (+ + ++) düzeyinde olmuştur. Modifiye QBC tüpü ile %0.03-%0.14 parazitemilerde (+), %0.15-%0.56 parazitemilerde (++) ve %0.56-%1.7 parazitemilerde (+++) değerler alınmıştır.

Abstract

The quantitative buffy coat method (QBC) is more sensitive than the conventional thick drop method since a greater volume of blood can be screened by it. QBC detection tubes are coated with acridine orange dye and an anti-coagulant such as potassium oxalate, sodium heparin or dipotassium EDTA. The separated and enlarged layers that can easily be differentiated in these tubes are light and dark red cells (erythrocytes), granulocytes, lymphocytes, monocytes and thrombocytes. Fluorescing erythrocytes infected with plasmodia as small as the tip of a needle are easily differentiated from uninfected ones. Gametocytes of Plasmodium falciparum are sickle shaped yellow bodies. Schizonts of P. vivax are identified by the detection of dark brown malaria pigments. Merozoites, growing in mature schizonts and encircled by dark brown pigments can be seen clearly. In our study, capillary tubes 7.5 cm long and 2 mm in diameter were used first and later tubes 10 cm in length and 0.3 mm in diameter were used. This increased the length 2.5 cm and narrowed the diameter six times. Blood samples from 237 patients with P. vivax parasitemia with values ranging between 0.03-1.7% were examined. The level of parasitemia was evaluated as (+), (++), (+ + +), and (+ + + +). The parasitemia values were as follows; 0.03-0.14% (+), 0.15-0.56% (++) and for 0.57-1.70%(+ ++).
