İmmun sistemi baskılanmış sıçanlarda saptanan Cyclospora sp. tanısında kullanılan yöntemlerin değerlendirilmesi

Özet

Son yıllarda yapılan pekçok araştırmada, uzamış ishalli olgularda saptanan, tanımlanamamış asid-fast pozitif bir organizma olan; önceleri Cyanobacterium olabileceği düşünülen Coccidian protozoonlardan Cyclospora sp. olduğu saptanmıştır. Çalışmamızda Cyclospora spp. laboratuvar tanı yöntemlerinin ve boyanma özelliklerinin değerlendirilmesi amacıyla, insan modeli olarak immün sistemi baskılanmış hayvanlardan yararlanılması planlanmıştır. Araştırmamızdaki 3 grup sıçandan 1.grupta 36 sıçanın 8 hafta süreyle, 2.grupta 10 sıçanın 12 hafta süreyle immün sistemleri deksametazon ile baskılanmıştır. Kontrol grubundaki 46 sıçana ise deksametazon verilmemiştir. Deney grubundaki sıçanlar 200 gr ağırlığında olup, her bir sıçana 1.5 mg (7.5mg/kg) deksametazon haftada 2 kez deri altına enjekte edilmiştir. Deney-1 grubunda 11 sıçanda (%30.55) Cyclospora pozitif saptanırken, Deney-2 grubunda 4 sıçanda (%40) Cyclospora olduğu görülmüştür. Toplam 46 sıçandan 15'inde (% 32.61) Cyclospora sp. gözlenmiştir. İmmün sistemi baskılanmayan kontrol grubundaki sıçanlarda ise Cyclospora spp. ookistlerine rastlanılamamıştır. Cyclosporiasis tanısında; otofluoresan verme özelliğinden, nativ-lugol inceleme, modifiye Ritchie çöktürme yöntemi, trikrom, Kinyoun karbol-fuksin, modifiye Ziehl-Neelsen, modifıye safranin, Giemsa, Gram Weigert, Löeffler'in alkalen metilen mavisi, May Grünwalt boyama yöntemleri ile sporulasyon yönteminden yararlanılmıştır. Modifiye safranin boyama yöntemi ile boyanan ookistlerin aynı tipte ve homojen olarak boyanması nedeniyle, diğer boyama yöntemlerinden daha iyi olduğu gözlenmiştir. Cyclospora pozitif dışkılara uygulanan fenol-auramine boyama yöntemi ile ookistlerin hem dış cidarının, hem iç yapısının daha parlak, belirgin fluoresan verdiği görülmüştür. Çalışmamızda İmmün sistemi baskılanmış kişilerde görülen ishal olgularında Cyclospora spp.'e yüksek oranda rastlanılabileceği, Cyclosporiasis tanısında otofluoresan özelliği, asit-fast boyama yöntemleri ve fenol-auramine boyama yönteminden yararlanılabileceği sonucuna varılmıştır.

Abstract

A number of reports over the last several years have linked a previously unidentified acid-fast organism with prolonged diarrhea in humans. Initially thought to be a cyanobacterium, the organism has been found to be a coccidian protozoan of the genus Cyclospora. Our aim in this study was to develop an immunosuppressed animal model for evaluation of diagnostic laboratory methods and to determine the characteristic staining of Cyclospora using various staining methods. A total of 92 rats used for 2 experimental groups and a control group. There were 36 rats in the first group; 10, in the second and 46, in the control group. In first group, the development of immunosuppression took 8 weeks in the first group and 12 weeks, in the second. For suppression of the immune response, we injected 1.5 mg (7.5 mg/kg) dexamethasone into the rats twice weekly. Cyclospora spp. were found in the stool specimens of 11 out of 36 (30.35%) rats in Group 1 and in 4 (40.00%) out of 10 rats in Group 2. Oocysts of Cyclospora spp. were not found in the control group. Therefore, it was suggested that it is more likely to find Cyclosporiasis in immunosuppressed patients. The auto-fluorescence method, native-Lugol examination, modified Ritchie concentration, and Kinyoun carbol-fuchsin, modified Ziehl-Neelsen, modified safranin Giemsa, Gram-Weigert, and May-Grunwaldt staining methods as well as Loeffler's techniques and the sporulation method have been accepted for identification of Cyclospora spp. It was found that the modified safranin method was better than other staining techniques since the oocysts were homogenously stained by this technique. In the phenol-auramine staining technique, oocysts in cyclospora-positive stool samples stained well with both the external wall and internal structure of oocyst fluorescing brightly. Therefore, it was concluded that this staining technique might be useful in detection of Cyclospora spp. We concluded that Cyclospora spp. may be found in the diarrheal illness observed in immunosuppressed patients so that the use of autofluorescence, acid-fast staining techniques and the phenol-auramine staining technique for diagnosis of Cyclospora spp. may be helpful
