Akne vulgaris patogenezinde Demodex folliculorium ve Propionibacterium acnes'in rolü

Özet
Akne vulgaris kıl folikülleri ya da yağ bezlerinin inflamatuar bir hastalığı olup, patogenezinde bir çok etken rol oynamaktadır. Akneli 78 hastanın sivilce içerikleri alınmış, Propionibacterium acnes için Gram preparait hazırlanmış ve anaerop besiyerine ekilmiştir. Aynı zamanda bu hastaların alın, yanak ve çene olmak üzere üç yüz bölgesinden ve sivilcelerinden deri yüzey biyopsi tekniği ile örnekler alınarak Demodex folliculorum yönünden incelenmiştir. Bu 78 hastanın 12(%15.4)'sinde Demodex folliculorum saptanmış; 62(%79.5)'sinde Propionibacterium acnes, 2 (%2.6)'sinde Propionibacterium granulosum ve 9 (% 11.5)'da P. acnes ve plazma koagulaz negatif stafilokok birlikte üretilmiştir. Demodex saptanan olgular Permetrinin %5'lik kremini ile tedavi edilmiştir. Bu çalışmada akne lezyonlardaki P. acnes ve D. folliculorum'un beraber bulunup bulunmadığına bakılmıştır.
Abstract
Acne vulgaris is an inflammatory disease of pilosebaceous and pilose follicles. Many factors play a role in its pathogenesis. Of 78 patients with acne, specimens of exudate were taken from their pimples, stained by the Gram method for Propionibacterium acnes and cultured anaerobically. Simultaneously, skin biopsies were taken from the foreheads, cheeks and chins and pimples of these patients for detection of Demodex folliculorum. In this study, we detected P. acnes alone in 62 (79.5%) patients, both P. acnes and plasma coagulation negative staphylococci in 9 (11.5%) patients, P. granulosum in 2 (2.6%) patients and D. folliculorum in 12 (15.4%) patients. It was observed that D. folliculorum disappeared in patients after the use of ointment containing 5% permethrin and all the lesions healed. The aim of this study was to determine whether P. acnes and D. folliculorum could be found in the same acne lesions.
