Neoplaziye bağlı gelişen omurilik hasarlı hastaların demografik özellikleri

Özet
Amaç: Bu çalışmada hastanemizde yatarak rehabilitasyon programına alınan neoplaziye bağlı gelişen omurilik hasarlı hastaların klinik ve demografik özelliklerini saptamak amaçlandı. Gereç ve Yöntem: Hastaların dosyaları taranarak retrospektif olarak incelendi. Çalışmaya 32 (18’si erkek, 14’ü kadın) neoplasiye bağlı gelişen omurilik hasarlı hasta alındı. Bulgular: Etiyolojide en sık rastlanan neden %3,2 ile metastatik tümörler, %21,9 ile epandimom, %18,8 ile astrositom ve %15,6 ile menengiom idi. Nörolojik seviye olarak %56,2 ile en fazla torakal bölge yaralanmaları vardı. Bunu %28,1 ile servikal ve %15,6 ile lumbosakral bölge izliyordu. ASIA sınıflamasına göre 5 (%15,6) hasta ASIA A, 1 (%3,1) hasta ASIA B, 1 (%3,1) hasta ASIA C, 13 (%40,6) hasta ASIA D ve 12 (%37,5) hasta kauda equina sendromu olarak değerlendirildi. Hastaların yatışları boyunca en sık izlenen tıbbi komplikasyon %65,6 ile nörojenik mesane bozukluğuydu. Hastaların %37,5’i ortez ile, %21,9’i bağımsız olarak ambule durumda ve %21,9’i tekerlekli sandalye düzeyinde, %18,8’i ise paralel bar içinde terapötik amaçlı ambule edilerek taburcu edildi. Sonuç: Rehabilitasyon ünitelerinde sıklıkla görülen neoplastik omurilik hasarlı hastalar travmatik omurilik yaralanmalı hastalardan farklı demografik özellikler göstermesine rağmen, rehabilitasyon sonuçları benzerlik göstermektedir
Abstract
Objective: The aim of this study was to evaluate the neoplastic spinal cord injured patients for their clinical and demographical data.Materials and Methods: The records of the patients were retrospectively analysed. Thirty-two patients (18 males, 14 females) with neoplastic spinal cord injury were included in this study.Results: The major etiological factors were metastatic tumor with 31.2%, epandimoma with 21.9% and followed by astrositoma with 18.8% and menengioma with 15.6%. The most common levels of neurological injury were the thoracic, cervical, and lumbar regions with the ratios of 56.2%, 28.1%, and 15.6%, respectively. The evaluations according to ASIA classification were ASIA A with 15.6%, ASIA B with 3.1%, ASIA C with 3.1%, ASIA D with 40.6% and cauda equina syndrome with 37.5% of the patients. The most commonly encountered complication during the hospital stay was neurogenic bladder with 65.6%. At discharge 37.5% of patients were ambulated with braces, 21.9% of patients were ambulated independently, 21.9% of patients were mobile with a wheelchair and 18.8% of patients were at the level of therapeutic ambulation at parallel bar. Conclusion: Patients with neoplastic spinal cord injury represent a significant proportion of spinal cord injury rehabilitation admissions and although have different demographics and injury characteristics, can achieve similar functional outcome as traumatic injuries.
