The effectiveness of carpal bone mobilization accompanied by night splinting in ıdiopathic carpal tunnel syndrome

Özet

Amaç: Karpal Tünel Sendromunda (KTS), Karpal Kemik Mobilizasyonu(KKM) ile birlikte uygulanan gece splintinin etkinliğini ölçmek ve sadecegece splinti uygulamasının etkinliğiyle karşılaştırmaktır.Gereç ve Yöntemler : Bu tek kör (değerlendiren) randomize kontrollüprospektif çalışmada, 40 hastaya elektronöromyograf ile hafften ortadereceye kadar olan KTS tanısı konmuştur. 1. grup hastalar (n=20)haftada 3 kez, toplamda 10 kez KKM tedavisi almış ve 3 hafta boyuncanötral volar gece splinti kullanmışlardır. İkinci grup hastalar (n=20) 3hafta boyunca sadece nötral volar gece splinti kullanmıştır. Bütünhastalar başlangıçta ve üçüncü ayda; ağrı için numerik ağrı skalasıyla(0-10), el ve pinch kavrama gücüyle, fonksiyonel durum ve semptomşiddeti için Boston Karpal Tünel Anketiyle (BKTA) ve elektronöromyografile değerlendirilmiştir.Bulgular: Birinci grupta tüm klinik değişkenlerde, median sinir distalduysal latans ve duysal sinir aksiyon potansiyeli ölçümlerinde düzelmelergözlenmiştir. İkinci grup hastalarda üçüncü ayda, sadece BKTAnınsemptom şiddeti ve gece/gündüz ağrısında iyileşme gözlenmiştir.Üçüncü ay pinch gücü ölçümü ve BKTA fonksiyonel durum ölçümünde1. grup hastalar, 2. grup hastalara göre daha belirgin düzelmegöstermiştir.Sonuç: Her iki tedavi şekli de semptomatik iyileşmede önerilebilir.Ancak, noninvaziv alternatif bir tedavi şekli olan KKM ve gece splintikombinasyonu, KTSde fonksiyon ve güç artışı bakımından daha iyi birtercih olabilir.

Abstract

Objective: The aim of this study was to investigate the effectiveness ofcarpal bone mobilization (CBM) and night splinting and to compareit with night splinting as a control group in the treatment of CarpalTunnel Syndrome (CTS).Methods: In the single-blind (assessor) prospective randomised controlledtrial, a total of 40 patients with mild to moderate CTS were diagnosed byelectroneuromyography (ENMG). In Group 1 (n=20), patients receivedCBM three times a week, total of 10 times, and used neutral volarwrist splint at night for 3 weeks. Patients in Group 2 (n=20) used onlyneutral volar wrist splint at night for 3 weeks. All of the patients wereassessed at baseline and at the 3 rd month with respect to pain intensityusing a numerical rating scale (0-10), handgrip and pinchgrip strength,functional status and symptom severity using the Boston Carpal TunnelQuestionnarie (BCTQ), and ENMG measurements.Results: There were improvements in all of the clinical variables, distalsensory latancy, and sensory nerve action potentials of the median nervein Group 1. In Group 2, there was improvement in only BCTQ symptomseverity and night/day pain intensity at the 3 rd month. Improvement inpinchgrip strength and BCTQ functional status were superior in Group1 when compared with that in Group 2 at the 3 rd month.Conclusion: Both of the treatment modalities may be recommendedfor symptomatic relief. However, CBM combined with splinting maybe a better choice for functional and strength amelioration as a non-invasive alternative treatment modality in CTS.
