Özet
Dört yıllık dönem içerisinde, adolesan yaş kümesinde yer alan üç adet filloides tümör olgusu saptadık. Yaşları 10, 13 ve 16 olan bu olguların ikisinde kitle eksizyonu, birinde ise radikal mastektomi uygulanmıştır. Makroskopik olarak papiller gelişimleri seçilebilen bu lezyonların histopatolojik incelenmesinde, stromal sellülarite hafif ya da orta derecede olup atipi saptanmamıştır. Filloides tümör, stromal selülarite artışı, lokal nüks ve malignité potansiyelleri nedeni ile fibroadenomlardan ayrı bir antitedir. Genellikle 4. ve 5. dekatlarda görülmekle birlikte, seyrek olarak adolesan yaşlarda da görülür. Adolesan çağda saptanan filloides tümür orta yaştakilere göre ayrı davranır; hızlı gelişir ve malignité potansiyeli oldukça düşüktür. Bu çalışmada, adolesan çağda saptanan 3 filloides tümörün özellikleri incelenmiş, bu yaş kümesinde görülen diğer meme kitleleri ile ayırıcı tanı özellikleri tartışılmıştır.
Abstract
Three cases of phylloides tumour of adolescent age were diagnosed during a four year period. The ages of the patients were 10, 13 and 16. Two of the cases were treated with tumour excision and radical mastectomy was performed in one. Papillary prolifera-tion of the lesions could be grossly detected. In histopathological examination, stromal cellularity was slight to moderate and no cellular atypia was encountered. Phylloides tumour is a distinct entity in comparison to fibroadenoma with its stromal cellularity, and potential of local recurrence and malignancy. Though it is a tumour of fourth and fifth decade, it can occasionaly appear in adolescent age. Phylloides tumour of young age beha-ves different from that of adult age with its rapid enlargement and low malignancy potential. The characteristics of phylloides tumour in adolescent age is evaluated with presentation of three case reports, and differential diagnosis with other breast lesions of this age group is discussed in this study.
