Özet

Amaç: Kistik Fibrozis (KF), hem çocuğu hem aile sistemini etkileyen kronik bir hastalıktır. Hastalığın getirdiği yükle birlikte ailenin ruhsal durumu etkilenebilmekte, aile işlevlerinde sorunlar yaşanabilmektedir. Baş etme becerileri, kişinin ruhsal uyumunu belirlemede önemlidir. Çalışmada, KF tanısı ile izlenen çocukların annelerinde psikiyatrik belirtiler, ebeveynlik tutumları ve baş etme becerilerinin değerlendirilmesi ve psikiyatrik belirtiler ile baş etme becerileri, ebeveynlik tutumlarının ilişkisinin incelenmesi amaçlanmıştır.Gereç ve Yöntemler: Çalışmaya KF tanısıyla izlenen 35 çocuğun annesi ve polikliniklere akut nedenlerle başvuran ve kronik hastalığa sahip çocuğu olmayan 40 anne alınmıştır. Annelerin değerlendirilmesinde Belirti Tarama Listesi, Başa Çıkma Tutumları Ölçeği ve Aile Hayatı ve Çocuk Yetiştirme Tutumu Ölçeği ve sosyodemografik bilgi formu kullanılmıştır. Bulgular: KF’li çocukların annelerinde Belirti Tarama Listesiyle belirlenen tüm puanlar kontrol grubundan yüksek bulundu. Bunun yanı sıra, bu annelerin aşırı koruyuculuk ve sıkı disiplin puanları da yüksekti. Başa Çıkma Tutumları Ölçeğinde soruna odaklanma ve duyguları açığa vurma, inkâr, dini olarak başa çıkma ve kabullenme alt ölçek puanları KF’li çocukların annelerinde kontrol grubundan istatistiksel olarak anlamlı düzeyde yüksek bulunurken; şakaya vurma, duygusal sosyal destek kullanımı ve plan yapma alt ölçekler puanları ise KF’li çocukların annelerinde daha düşük olarak saptandı. KF’li çocukların annelerinde Belirti Tarama Listesi-Genel Belirti Düzeyi kesim noktası “1” olarak alındığında, psikopatolojiyle uyumlu puan alan KF’li çocukların annelerinde, ev kadınlığı rolünü reddetme ve evlilik çatışması puanları psikopatolojiyle uyumlu puan almayan annelere göre yüksek bulundu. Belirti Tarama Listesi- Genel Belirti Düzeyine göre muhtemel psikopatolojisi olan KF tanılı çocukların annelerinde Başa Çıkma Tutumları da değerlendirildi. Bu annelerde zihinsel boş verme, inkar, duygusal sosyal destek kullanımı ve uyum bozan başa çıkma yollarına ilişkin puanları istatistiksel olarak daha yüksek düzeydeydi.Sonuç: Çocukları KF tanısı ile izlenen annelerde hem baş etme becerileri ve evlilik ilişkisi hem de ruhsal sağlık olumsuz olarak etkilenmektedir. Ülkemizde KF’li çocuğa sahip annelerin baş etme becerilerini geliştirecek ve psikososyal destek sağlayacak çalışmalara gereksinim bulunmaktadır

Abstract

Objective: Cystic fibrosis (CF) is a chronic disease affecting both the child and the family system. The mental state ofthe family can be affected with the burden brought on by the disease, and family functioning can become problematic.Coping abilities and strategies are important in determining the psychological adjustment in difficult situations. The aimof this study was to evaluate psychiatric symptoms, parental attitudes and coping strategies in mothers of children withCF and to examine the relationship of psychiatric symptoms with parental attitudes and coping skills. Material and Methods: The study included the mothers of 35 children with CF and of 40 children with acute medicalproblems and no chronic diseases. The Symptom Checklist-90-Revised, the COPE inventory, the Parent AttitudeResearch Instrument and the sociodemographic information form were used in the evaluation of the subjects. Results: All scores on the Symptom Checklist-90-Revised were higher than the control group in mothers of childrenwith CF. Additionally; these mothers had higher scores on dependency and strictness/authoritarianism on the Parent Attitude Research Instrument. In the COPE inventory; focus on and venting of emotions, denial, religious coping and acceptance subscalescores were higher, whereas humor, use of instrumental social support and planning subscale scores were lower in mothers of childrenwith CF. When mothers of children with CF were categorized according to the Symptom Checklist-90-Revised Global Severity Index witha cut-off score of “1”; mothers with scores higher than 1 suggesting psychopathology had higher scores for rejection of the homemakingrole and marital conflict and coping strategies such as mental disengagement, denial, use of emotional social support, and ways ofmaladaptive composites. Conclusion: Coping abilities, marital relationship and mental health are negatively affected in mothers of children with CF. There is a needfor further studies and developing programs that will improve the coping skills and provide psychosocial support for mothers of children withCF in our country
