Bir hastane polikliniğinde tedavi gören çocuklarda rutin aşıların yapılma oranları

Özet

Amaç: Bu çalışmada istanbul'un merkezinde bulunan ve toplumun düşük veya çok düşük sosyo-ekonomik gruplarının başvurduğu Şişli Etfal Eğitim ve Araştırma Hastanesi çocuk hastalıkları polikliniğinde çeşitli hastalıklar nedeniyle tedavi edilen çocuklarda rutin aşılar (BCG, difteri-boğmaca-tetanoz, kızamık, polio ve hepatit B) ile aşılanma oranları ve bunu etkileyen faktörler araştırıldı. Yöntem: Çalışmaya 2002 yılının ilk üç ayında Şişli Etfal Eğitim ve Araştırma Hastanesi Çocuk Hastalıkları Polikliniği'nde ayakta tedavi gören 1-7 yaş arasındaki çocuklar alındı; BCG, DBT, polio, kızamık ve hepatit B aşılarının yapılma oranları öğrenildi. Çalışmaya alınan 325 çocuğun yaş ortalaması 5.2±2.4 yıl (ortalama±standart sapma) ve erkek kız oranı 1.20 idi. Bulgular: BCG, kızamık, DBT, polio ve hepatit B ile tam aşılanma oranları sırasıyla %66.4, %46, %34.1, %33.8 ve %20.1 idi. ince¬lenen 325 çocuktan %56'sının dört veya beş aşısı eksikti, 14 çocuğun bütün aşıları tamdı. 8 (%2.4) çocuk ise hiç aşılanmamıştı. Aşıların eksik veya hiç yapılmaması %52.4 oranında aşılama döneminde hastalığa, %46.4 oranında ailenin bilgisizliğine, %1.2 oranında da zaman bulamama ve yan etkilerden çekinmeya bağlıydı. 325 çocuğun %60.4'ü daha önce en az bir kere herhangi bir sebeple bir sağlık kuruluşuna başvurmuştu. Sonuç: Bu sosyal grupta rutin aşılama oranı çok düşüktür; kaçırılan fırsatlarının değerlendirilmesiyle bu oran önemli ölçüde arttırılabilir.

Abstract

Aim: The aim of this study was to evaluate the administration rates of BCG, diphteria- pertussis- tetanus, measles, polio and hepatitis B vaccines and the factors affecting these rates in 1-7 year-old children applying to the Outpatient Pediatric Clinic of Şişli Etfal Research and Education Hospital which is located in central Istanbul and has a patient profile largely composed of patients from low and very low socio-economic status. Methods: All of the 1-7 year- old patients applying to the Pediatric Clinic of the Sisli Etfal Research and Education Hospital during the first three months of 2002 were included in the study. Vaccination history for BCG, DPT, polio, measles and hepatitis B was investigated. The average age of the 325 patients included in the study was 5.2±2.5 years and the male:female ratio was 1.20. Results: Proper vaccination rates for BCG, measles, DPT, polio, and hepatitis B were 66.4%, 46%, 34.1%, 33.8% and 20.1%, respectively. Four or five vaccines were found to be missed in 56% of the 325 patients and full vaccination schedule was completed only in fourteen patients. Eight (%2.4) infants in the study group, had not received any vaccination. The reasons for missing or no vaccination were; being ill at the vaccination period for 52.4% of cases, lack of information of the family in 46.4% of the cases; lack of time and fear of side effects in 1.2% of the cases. It was also found that 60.4% of the 325 patients had had at least one admission to a health care centre in the past. Conclusion: Administration rates of the routine vaccines are very low for the study group in question. Taking better advantage of all the opportunities to initiate the vaccination schedule might significantly increase the vaccination rates.
