Kronik Hepatit C hastalarında Hepatit B enfeksiyonu ile karşılaşma sıklığı: Geriye dönük bir çalışma

Özet

Amaç: Kronik hepatit C hastalarının hepatit B enfeksiyonu ile karşılaşmış ve aşılanmış olma oranlarının belirlenmesi amaçlandı.Yöntem: Ocak 2001-30 Haziran 2008 tarihleri arasında başvuran ve anti-HCV (Hepatit C virüs antikoru) pozitif 223 hastanın HBsAg (Hepatit B yüzey antijeni), anti-HBs (Hepatit B yüzey antikoru), anti-HBcIgG (Hepatit B kor IgG antikoru), anti-HBcIgM (Hepatit B kor IgM antikoru), HBV-DNA (Hepatit B virüs DNA) ve HCV-RNA (He- patit C virüs RNA) serolojik belirteçleri bilgisayar veri tabanından saptandı. Bulgular: HBsAg, anti-HBs ile birlikte anti-HBc IgG ve izole anti HBcIgG pozitiflikleri sırası ile %4, %14.9, %17 idi. Hepatit B enfeksiyonu ile karşılaşmış olma ve aşılanmış olma oranları sırası ile %31.9, %26.2 idi. Bölgemizdeki kronik hepatit C hastalarının hepatit B ile karşılaşma ihtimali yüksek, ancak aktif bağışıklanmış olma oranı düşük bulundu. Sonuç: Kronik hepatit C hastalarında; kronik karaciğer hastalığının ilerleyişini olumsuz yönde etkileyen ve aynı bulaş yollarını kullanan hepatit B virüsünün varlığı serolojik yöntemle mutlaka araştırılmalıdır. Bu hastalar ve aile hekimleri hepatit B aşısı ile aşılanmaları konusunda uyarılmalı ve yönlendirilmelidir.

Abstract

Objectives: To determine the immunization status against and the encounter rate with hepatitis B virus in patients with chronic hepa- titis C infection. Methods: A total of 223 patients positive for anti-HCV were screened retrospectively using the computer based patient records between January 1, 2001 and June 30, 2008 in terms of hepatitis B surface antigen (HBs Ag), hepatitis B virus surface anti- body (anti-HBs), hepatitis B virus core IgG antibody (anti-HBcIgG), hepatitis B virus core IgM antibody (anti-HBcIgM), hepatitis B virus DNA (HBV-DNA), hepatitis C virus RNA (HCV-RNA). Results: The rates of HBsAg, anti-HBs and anti-HBc IgG, and iso- lated anti-HBc IgG seropositivity were 4.0%, 14.9% and 17.0% respectively. The rates of infection with and the active immuniza- tion against hepatitis B virus were 31.9% and 26.2%, respective- ly. Consequently, patients with chronic hepatitis C infection in our region were found to be highly prone to co-infection with hepa- titis B virus, and less likely to have active immunization against it. Conclusions: Active immunization of sero-negative patients should be regarded as a healthcare priority because hepatitis B co- infection has significant adverse effects on disease progression in patients with chronic hepatitis C infection. These patients should be warned about it and directed to be vaccinated with hepatitis B vaccine by family physicians.
