Emergency severity index triaj sistemi ve bir üniversite hastanesi acil servisinde uygulama deneyimi

Özet
Amaç: Emergency Severity Index (ESI) acil müdahale gerektiren ve bekletilmesi riskli hastaları tespit ettikten sonra kaynak tahmini üzerinden hastanın triaj kategorisini belirleyen, bunun için algoritmada 4 temel karar aşaması bulunan kullanımı basit 5 seviyeli bir triaj sistemidir. Amacımız ESI triaj sistemi hakkında bilgi vermek ve sistemin acil servisimizde kullanılmaya başlanması sürecindeki deneyimlerimizi paylaşmaktır. Gereç ve Yöntem: Gülhane Askeri Tıp Akademisi (GATA) Acil Tıp Anabilim Dalı’nda Kasım 2009 tarihinden itibaren ESI triaj sistemi kullanılması kararlaştırılmış ve Agency for Healthcare Research and Quality (AHRQ) ile yazışılarak gerekli eğitim materyallerine ulaşılmıştır. İki günlük olgu örneklerinin ağırlıkta olduğu bir eğitim programı oluşturularak, acil serviste görev yapan 34 hemşireye 3 grup halinde ESI eğitimleri verilmiştir. Eğitimlerde ana kaynak olarak ESI Uygulama El Kitabı kullanılmıştır. Eğitimi ilgi çekici hale getirmek için triaj algoritmalarını gösteren posterler ve Triaj Oyunu olarak isimlendirilen özel bir eğitim tekniği kullanılmıştır. Eğitimin etkinliği 10 olgudan oluşan ön/son değerlendirme ve 30 olgudan oluşan pre/pos test yöntemi olmak üzere 2 farklı metod kullanılarak değerlendirilmiştir. Bulgular: Triaj kategorisini doğru belirleme oranlarının pre/post test yönteminde %30’dan (9.52±2.53) %80’e (24.88±3.01) yükseldiği saptanmıştır. Ön/son değerlendirmede ise eğitim öncesi triaj kategorisini doğru belirleme oranı %40 düzeyinde iken, eğitim sonunda bu oran %76’ya ulaşmıştır. Sonuç: ESI’nin kolay öğrenilebilir pratik bir sistem olduğu ve acil servis hemşirelerine yönelik düzenlenen olgu örneklerinin ağırlıkta olduğu 2 günlük eğitimin ESI triaj sistemini öğretmede etkin olduğu sonucuna ulaşılmıştır.
Abstract
Objective: Emergency Severity Index (ESI) is a simple to use, five-level triage instrument that categorizes emergency department patients in four decision points by evaluating both patient acuity and resources. Aim of this study was to introduce the ESI triage system and sharing the implementation experience of it in a university hospital emergency department. Methods: We planned to use ESI triage system in Gulhane Military Medical Academy Department of Emergency Medicine at November 2009. Education materials were provided from Agency for Healthcare Research and Quality (AHRQ). A two day long case based training program was planned for ESI education. We included 34 emergency nurses to training program in 3 seperate groups. ESI Implementation Handbook used as the primary source for training. Triage algoritm posters and triage games were prepared to make education more interesting. We evaluated the efficiency of training by using two different method: Pre/post evaluation and pre/post test method. Results: Determining the correct triage level was increase from 30% (9.52±2.53) to 80% (24.88±3.01) in pre/post test method which include 30 cases and 40% to 76% in pre/post evaluation method which include 10 cases. Conclusions: We concluded that ESI is simple to learn, practical system for emergency triage and two day case based training program was effective to teach it.
