Özet

Rektus kılıf hematomu, epigastrik arterlerin veya rektus kas liflerinin yırtılması nedeni ile kanın rektus kılıfı içinde toplanması sonrası gelişen karın ağrılarının nadir bir nedenidir. Rektus kılıf hematomunun uygun tedavi seçimi ve hastanın hastaneye yatırılma kararı hemodinamik bozulmanın derecesi, koagülasyon durumu ve abdominopelvik bilgisayarlı tomografideki hematomun yaygınlığına dayanır. Bu makalede acil servise ani başlangıçlı karın ağrısı ile başvuran, sonrasında varfarin doz aşımına bağlı spontan rektus kılıf hematomu tanısı alan 78 yaşındaki kadın sunulmaktadır. Makalemizde ayrıca acil servislerde rektus kılıf hematomlarının tanınması ve uygun şekilde yönetilmesi de tartışıldı.

Abstract

A rectus sheath hematoma occurs an accumulation of blood in the sheath of the rectus abdominis, secondary to rupture of an epigastric arteries or their branches or rectus muscle tear. The choice of effective treatment of rectus sheath hematoma and the decision to admit the patient to hospital depends on the degree of haemodynamic compromise, coagulation status as well as the type of the hematoma on abdominopelvic computerized tomography scans. In this article, we report a case of spontaneous rectus sheath hematoma presenting with acute abdominal pain associated with a palpable abdominal wall mass. Furthermore, diagnosis, classification, and management of rectus sheath hematoma in the emergency room are discussed.
