Paraözefageal herniye bağlı gastrik volvulus: olgu sunumu

Özet

Gastrik volvulus nadir görülen ve hayatı tehdit edebilen bir durumdur. Tedavisi acil cerrahi girişim gerektirir. Bu yazıda,acil servise epigastrik karın ağrısı, bulantı ve şiddetli kusma şikayetleri ile başvuran 85 yaşında kadın hasta, literatürlereşliğinde sunuldu. Hastanın yapılan incelemeleri sonucu pa-raözefageal herniye bağlı gastrik volvulus tanısı konarak acil laparaskopik cerrahisi yapılmış, ameliyat sonrası dördüncügünü sorunsuz olarak taburcu edilmiştir. Gastik volvulus, öz-gül olmayan abdominal semptomlarla seyrettiğinden tanısıgecikebilir veya atlanabilir. Radyolojik görüntüleme yöntemle-rinden akciğer grafisi ve torakoabdominal bilgisayarlı tomog- rafi tanıya yardımcıdır. Tedavide acil laparaskopik cerrahi altınstandarttır.

Abstract

Gastric volvulus is a rare and life-threatening condition, and treatment requires emergent surgery. This is a case report of an 85-year-old female patient that presented to the emergency department with epigastric pain, nausea, and intractable vom- iting. The patient was diagnosed with gastric volvulus due to paraesophageal hernia and subsequently underwent emergent laparoscopic surgery. She was discharged on postoperative day 4 and recovered without any complications. Gastric volvulus may present with non-specific abdominal symptoms, which may de- lay diagnosis or contribute to misdiagnosis. Chest X-ray and other radiological imaging techniques such as computed tomography are helpful in correctly diagnosing gastric volvulus, and emergent laparoscopic surgery is the gold standard of treatment.
