Ailesel Akdeniz Ateşi (FMF) düşünülen olgularda MEFV gen mutasyonları sıklığının incelenmesi

Özet

Amaç: Ailesel Akdeniz Ateşi (FMF) MEFV genindeki mutasyonların neden olduğu otozomal resesif bir hastalıktır. Bu çalışmada bir grup Türk'te, MEFV geninde sıklıkla rastlanıldığı bildirilen E148Q, P396S, F479L, M680I (G/C), M680I (G/A), I692DEL, M694V, M694I, K695R, V726A, A744S, R761H mutasyonlarının frekansı ve yüzdeleri incelenmiştir. Yöntem: FMF ön tanısı almış toplam 98 hastada MEFV geninde 12 mutasyon araştırıldı. Hastaların yaş ortalaması ± SD, 30 ± 12 yıl idi. MEFV genindeki mutasyonlar, ticari kit kullanılarak polimeraz zincir reaksiyonu (PCR) ve ters-hibridizasyona dayanan strip-assay ile saptandı. Bulgular: Çalışılan toplam 98 hastadan 67'sinde MEFV gen mutasyonları bulunmuştur (% 68,36). Allel frekansları yüzdeleri açısından incelendiğinde; M694V (%46,26), E148Q (5616,41), V726A(%13,43), M680I (G/C) (565,97), R761H (%2,98) bulunmuştur. P396S (%1,49) F479L(%1,49), A744S (%0,74 ), allellerinin oldukça düşük frekanslarda olduğu belirlenmiştir. M680I (G/A), I692DEL, M694I ve K695R mutasyonlarına ise rastlanmamıştır. Sonuç: Türk Toplumu'nda yapılan diğer çalışmalarda görüldüğü gibi, hastalarımızın büyük çoğunluğunda M694V mutasyonuna rastlanmıştır. Çalışmamızda en sık rastlanan ilk dört mutasyon türü yapılmış diğer çalışmalarla uyumludur ancak sıklığı farklı bulunmuştur. Türk Toplumu'nda daha önce bildirilmiş mutasyon türlerinde 12 mutasyonun tümünün sıklığını bildiren çok az sayıda çalışma mevcuttur. Bu açıdan sonuçlarımızın ileride yapılacak fenotip-genotip korelasyon çalışmalarına yol gösterici olabileceğini düşünmekteyiz.

Abstract

Objective:Familial Mediterranean Fever (FMF) is an autosomal recessive disease caused by mutations in the FMF gene (MEFV). In this study, we evaluated the frequency and percentage of most common known twelve mutations (E148Q, P396S, F479L, M680I (G/C), M680I (G/A), I692DEL, M694V, M694I, K695R, V726A, A744S, R761H) of MEFV gene in a group of Turkish individuals. Methods: A commercial kit assay for the identification of MEFV gene mutations based on PCR and reverse-hybridization was used to investigate 12 mutations of the MEFV gene in 98 patients (mean age: 30+12years). Results: Sixty-seven patients (68.6 %) were found to have one of the MEFV mutations: M694V (46.26%), E148Q (16.41%), V726A (13.43%), M680I (G/C) (5.97%) and R761H (2.98%). Very low allele frequencies were observed in P396S (1.49%), F479L (1.49%), A744S (0.74 %) type mutations, while the M680l(G/A), I692DEL, M694I and K695R type mutations were not detected. Conclusion: The M694V mutation is the most frequent one among our study group similar to published estimates from Turkey. In addition, the frequency of the most common four mutation types, which were detected in this study were similar but with different percentages when compared to the previous estimates. There are not many published estimates on the 12 types of MEFV gene mutations of Turkish patients, therefore our results may be beneficial for phenotype-genotype correlation studies.
